A1. Please give us a complete and honest assessment of anyweakness(es) in your application that you believe prevented you from gaining admission to a medical or dental school when you applied. Please note whether you contacted any admissions officers at medical or dental schools for a review of your application and explain what they told you.
A2. Please explain to the committee why you ultimately decided not to apply to medical or dental school after completing your committee interview. What factors went into your decision?
B. Please give us a complete and thorough update on everything you have done in the last year to improve your application for this cycle. Make sure you specifically address weaknesses you listed aboveif you applied or factors that went into your decision not to apply if you did not. We are not asking you to redo the supplemental information form you completed last year, but we do want to know what you would be adding to that form since last year.
