Terms and Conditions for Private occupational medicals

1 The role of the GP in providing private occupational medicals is to provide factual information to the decision maker (DVLA etc). We will not embellish or “improve” what we report. We do not cut anything out and you should be aware that our reports can equally well lead to the loss of your licence as to the renewal.

2 Some of our reports will be returned to us by the decision making agency for further questions. This can lead to unavoidable delays. Please leave enough time between coming to us for your report and the running out of your licence. 4 weeks are the absolute minimum we recommend.

3 Sometimes we recommend further investigations, e.g. optician's reports. This can lead to delays. See (2). Please plan for these delays.

4 Medical reports of this kind are private and not NHS work. You are responsible to pay us the fee. Sometimes your employer will refund you. Whether your employer does so or not does not remove your responsibility to pay us. We will not issue any reports without having received payment. Delays introduced by your failure to pay are your responsibility alone.

5 Failure to attend at pre-booked private appointments for medical reports will lead to charges of up to 50% of the report fee, reflecting the time wasted by your failure to attend. Such charges will need to be paid in full prior to the issuing of the report.

6 NHS medical work and medical emergencies will always take a priority over private work done by this surgery. Please see (2).

7 It is always possible that in the course of the medical examination or review of your medical records something becomes obvious which might affect other licenses of yours (e.g PCV licence). We will be obliged to give you advice accordingly and on occasion will have the ethical obligation to discuss such findings with the DVLA (or other licence giving agency) in order to ensure public safety.
