In this section, the Proposer must include details of its experience.

1Proposer Company Profile

Instructions: The Executive Summary may be submitted separate from the template.

1.1Executive Summary

The Proposer shall submit a one (1) tofive (5) page executive summary intended to provide the City with a brief history of the Proposer, including how it is organized, and how its products, services, and resources will be used to meet the City’s needs.

The Proposer shall describe its vision of how it sees itself in the marketplace with a five-year outlook,including:

  1. A statement of strategic direction
  2. Plans, with time frames included, enhancing your service offerings and to what degree will these enhancements impact customers
  3. Vision for the product or product roadmap for the next two to five years
  4. Customer role in development of the product

2Company Experience

Instructions: Provide all relevant information regarding the general profile of the Proposer. Written essay responses within the Reference sections must be under 500 words. Existing business relationships with the City must be described within a page.

The Proposer shall submit detailed information on its references in the template below.References may be contacted to verify that the Proposerhas experience in design, development, and implementation of public-sector solutions similar in size, complexity, and scope to this procurement per the minimum qualifications table in Template A. 2.2Existing Business Relationships with the City

The Proposer shall describe any existing or recent business relationships (within the last five (5) years) that the Proposer or any of its affiliates or proposed Subcontractors has with City departments and divisions. Please include if the Proposer has (active or ended) contracts with the City.

3Staff Experience

3.1Staff Experience

Instructions: Provide a list of proposed personnel identifying each key person on the project team including the role each will play in the project. Provide a description of the experience and qualifications of the project team members, including brief resumes and the role each played on previously referenced projects listed in Section 2 above. Proposers shall provide a summary of each key member’s high-level responsibilities.

3.2Project Organization and Staffing Chart for this project

Instructions: Proposers must provide a proposed organization chart showing both the Proposer staff. The organization chart must include all key Proposer personnel.

3.3Project Team’s Approach to Collaborating with other City Contractors

Instructions: Describe the proposed approach and procedures regarding collaborating and working in partnership with the other City contractors supporting the System project (e.g., Vendors of source systems, or possibly other System functionality implementers). Proposers should expect to be working on coordinated work streams with inter-vendor dependencies throughout the engagement, and all Proposers will be expected to contribute to project and program-level management of planning, risk mitigation, and reporting.
