Sydney Excursion Note 2

19th June 2014

Dear Parents/Guardians

Activities for the Sydney Excursion have now been booked. An itinerary and permission note follows.

As places on the excursion are limited, priority will be given to those who have paid and provided their permission note first. Parents are reminded that, as outlined in the school’s discipline code, students who have zero points on their Excursion Licence will be unable to attend.

Before completing the permission notes we ask you impress upon your son/daughter the high expectations of conduct Duval High School teachers have of their students when representing their school.

Full school uniform is required on days 2 of the excursion. Alcohol and tobacco products are not permitted. Students who do not meet the required standards of behaviour on the excursion may be sent home at parents’ expense.

I hereby consent to my son/daughter/ward ______(full name) participating in the Year 10 Sydney excursion approved by the Principal.

The excursion will depart from Duval High School at 7.30am Tuesday December 2nd 2014 and return to Duval on Friday 5th December 2014 at 4.30pm. It will involve travel by coach and ferry.

The organising teacher is Ms C Simes. Students will be accommodated at the YHA at The Rocks.

I understand that all possible care will be taken to ensure the welfare of students. However, I realise that circumstances could arise which necessitate a teacher making decisions in relation to my child’s/ward’s welfare without the opportunity to refer to me. I therefore agree to abide by such decision made in good faith. In the event of accidents or illness, I accept responsibility for his/her return, and agree to pay any costs incurred.

  • I understand that my child/ward will be given the opportunity to swim at Wet’n’Wild. His/Her swimming ability is:
