Acceptance of this order must be by return of a signed copy of this form by an officer of Surgoinsville Middle School Athletic Department. Do not attach additional terms without prior telephone clearance from Surgoinsville Middle School Athletic Department.

The Surgoinsville Middle School Athletic Department will provide the purchaser with advertisement in the form of a 2.5’x 2.5’ printed banner. The banner would hang in visible sight for all 2016-2017 Basketball, 2016 -2017 Baseball and 2017-2018 Football home games at Surgoinsville Middle School.

2016 SMS Basketball

The Surgoinsville Middle School basketball teams will once again be participating in a banner fundraiser for the 2016-2017 season.

This years fundraiser will be slightly different from years past. For the 2016-17 season, companies will be purchasing a 2.5’x 2.5’ banner. This banner will be visibly displayed at all 2016-17 home basketball games, all 2016-17 home baseball games, and all 2017 home football games. The advertising contract will run from 11-7-2016 through 10-1-2017. The cost of the advertisement will once again be $100.00. However, any company that has previously purchased a banner with our program over the last few years will be able to purchase the advertisement at a discounted rate for this season. Returning companies or individuals may purchase the advertising for $75.00.

All members of the basketball programs will be asked to sell a minimum of 2 banners for this season. In this packet, you will find two copies of the advertising contract that any individual or company must sign. You will also find a list of every company or person that has previously purchased a banner from our program. Remember these people will be able to purchase the 2016-17 advertising rates at a discounted price. I recommend that athletes start with these companies and then begin adding new. Additional copies of the advertising contract may be found on our school basketball site.
