Directions

For each area, decide the level at which the employee performed for this evaluation period and check the appropriate box by clicking on it. Scores range from 5 – Outstanding, 4 – Above Expectations, 3 – Meets Expectations, 2 – Below Expectations, and 1 – Unsatisfactory. Comments must be provided for each area where there is a score of 5 – Outstanding or 1 – Unsatisfactory.

Both the evaluator and employee should complete an evaluation. The evaluator shall then meet with the employee, review both evaluations with the employee, provide the employee a copy of the evaluation findings and file the signed original copies in the employee’s personnel file. Evaluators may ask for input from work-level supervisors and consider the information as one source in the evaluating process.
