OUTCOME DOMAIN 1: MOTIVATION

Readiness to change survey prompt: “Are you currently planning to make any changes to keep yourself healthy or improve your health?”

  1. Diet and Nutrition
  2. Exercise
  3. Tobacco Use
  4. Weight Management
  5. Stress

Response options were the same for A-E and coded for analysis as follows:

  1. Not interested
  2. Yes, in the next 6 months
  3. Yes, in the next 30 days
  4. I have recently made a healthy change
  5. I am already maintaining healthy habits

These five survey items used the same survey prompt, presented in a grid with topics A-E in rows and response options (1-5) in columns.

OUTCOME DOMAIN 2: BEHAVIOR

Vigorous Activity survey prompt: “In a typical week, how many times per week do you do VIGOROUS leisure-time physical activities for AT LEAST 10 MINUTES that cause HEAVY sweating or LARGE increases in breathing and heart rate?”

Response option set was preceded with “# Times per Week” and coded for analysis as follows:

  2. 1
  3. 2
  4. 3
  5. 7
  6. 5
  7. 6
  8. 7
  9. 8 or more

Moderate Activity survey prompt: “In a typical week, how many times per week do you do LIGHT OR MODERATE leisure-time physical activities for AT LEAST 10 MINUTES that cause ONLY LIGHT sweating or a SLIGHT to MODERATE increase in breathing or heart rate?”

Response option set was preceded with “# Times per Week” and coded for analysis as follows:

Simple Carb Intake survey prompt (following prompt above): “Fried food, high-fat foods, or simple carbs. Examples: Fried chicken, fried fish, bacon, french fries, potato chips, corn chips, doughnuts, creamy salad dressings, white bread, white rice, pasta, and foods made with whole milk, cream, cheese, or mayonnaise”

Response option set was preceded with “# servings per day” and coded for analysis as follows:

Social Support survey prompt: “How often do you get the social or emotional support you need?”
