SENIOR CLASS TRIP TO BLACK BEAR LAKE COUNTRY CLUB

trip at Black Bear Lake Country Club, located in Clarksburg, NJ from 9:00 am to 2:00 pm on Tuesday, June 7, 2016. It is agreed that the student will accept the authority of the administrators/teachers in charge and that neither the school nor any school authority will be held responsible for any injury/injuries, he/she might suffer while on the trip. I also understand that the student is responsible for the same standard of conduct required in the school as set forth in the student handbook. It is understood that this student will take school transportation to and from this activity. Students may not use private transportation and will be forbidden to attend the event without submitting this permission slip by FRIDAY, MAY 13.

Attendance will be taken on all buses, therefore, students may not switch buses.

**FOOD AND DRINK WILL BE PROVIDED ALL DAY

**Cost of this trip is $38 per senior. Cash or Money Order Only. NO REFUNDS.

**Remaining Patriot Card Credit may be used. Indicate if you are using Patriot Card Credit.
