Supervisor Job Description Revised 01/07/2016

Directly reporting to the Manager with oversight from the Program Project Manager and Program Director, the Supervisor is responsible for providing day-to-day supervision and oversight to the Direct Support Professionals working in their location. Primary responsibilities are to ensure that employees are providing exemplary care, are equipped, exceeding the standards of care and are fiscally responsible.

Duties:

  • Provides hands on training to Direct Support Professionals. Role modeling appropriate behavior and leading by example. Provides clear instructions.
  • Attend Individual plan meetings and to be a resource and provide information on the individuals.
  • Completes staff assignments ensuring that assignments include the appropriate supervision and chores. Ensure ratios are met
  • Ensure active treatment and engagement is occurring.
  • Provide med pass observation in waiver or med pass direction in ICF locations. Conducts the med change.
  • Responsible for providing cash and retrieving receipts for outings and purchases.
  • Plans and ensures appropriate shopping – grocery, clothing, emergency supplies, housing needs are completed.
  • Plans activities and outings with Direct Support Professional input.
  • Ensure plans are being followed, review documentation
  • Ensures that Unusual Incident reports are completed and reported when on duty. Responsible for faxing witness statements.
  • Approves payroll for Direct Support Professionals whenever on duty.
  • Responsible for the staffing schedule and position control. Will ensure vacant shifts are posted, approved, vacation requests are processed, and filling staffing call-offs and needs while on duty.
  • Work with Managers to ensure transfers and separation information is processed.
  • Ensures the maintenance needs of the house are reported and addressed.
  • Ensures cleanliness of the house and that the home is decorated appropriately.
  • Reports staffing issues to Manager and Human Resources. Follows up by providing disciplinary actions and commendations.
  • Ensures that the staff provide family style dining.
  • Ensures safety drills – fire and tornado – are completed timely and that any issues are reported.
  • Provides new hire training.
  • Conducts employee interviewing.
  • Identify training needs for the home and works with the Manager to plan monthly staff meetings.

Minimum Qualificaitons:

  • High School Diploma or GED.
  • Experience as a Direct Support Professional.
  • Med Pass Certification
  • Computer knowledge, smart phone skills and ability to email, type, use Microsoft Word and internet.
  • Valid Ohio driver’s license with no more than four points, eligibility under our insurance carrier. Maintain an acceptable driving record adhering to all driving laws and Rose-Mary Center regulations pertaining to driving.
  • Read and write English.
  • Ability to work and lead a team, collaborate with co-workers, managers, and program staff via open communication in a positive and constructive manner.
  • Must have a cooperative, caring nature, and ability to work in a stressful situation in a calm therapeutic manner.

Physical Requirements:

  • Lifting 50 to 100 pounds.
  • Transferring individuals from (with or without mechanical lifts) from bed to wheelchair, wheelchair to bed, in and out of shower/bath, and in and out of vehicles.
  • Assisting with hygiene, including bathing/showering, shaving, tooth brushing, and assisting with incontinence care.
  • Bending, twisting, standing, climbing, and kneeling
  • Pushing wheelchairs
