STEUBEN COUNTY SUPERVISION OF PROFESSIONALS

POLICY: The Agency willmaintain quality of care standards and ensure proper administrative oversight of contracted/employed licensed professionals.

PURPOSE: To ensure that supervision of professionals is conducted throughout the continuum of care and is an integral part of the quality improvement program.

PROCEDURE:

Supervisory personnel are employed by the agency to assure quality of patient care services. Supervision includes:

  • ongoing review of cases and delegation of assignments;
  • in-home visits, at least annually, to direct, demonstrate and evaluate the delivery of LHCSAservices for patients;
  • in-home visits, at least annually, to direct, demonstrate and evaluate the delivery of Early Intervention Service Coordination;
  • clinic supervisory observations may also be conducted;
  • provision of clinical consultation, if applicable;
  • professional guidance on agency policies and procedures;
  • on-going monitoring of contracted personnel to ensure compliance with contractual agreement.

Deficiencies in care/services are brought to the attention of the professional and amendments made to the record as necessary. Corrective actions are implemented and tracked. Identification of deficiencies is addressed on a one-on-one basis with the professional. Follow-up is accomplished either through focused documentation review or additional on-site visits.

Complaints and/or incidents are channeled through the complaint process.

Breaches in the standards of practice and/or policies and procedures, unprofessional conduct, and/or exceeding the scope of licensure will be reported to the Director/Commissioner, and may be reported to the Human Resources/Personnel Department, and NYS Board of Education –Division of licensure, and may be grounds for progressive discipline and/or termination of contract.

Supervisory Oversight:

  • Patient centered case conferences are held as needed, according to workforce experience, education needs and agency needs.
  • Prospective review of documentation and caseloads.

Supervisory personnelwill conduct reviews of all projected scheduling of patient visitsand assignment to staff. This process ensures adequate dissemination of patient care loads among staff, and the delivery of quality care which is in accordance with regulations, standards of practice and physician’s orders.
