Sunderland Youth Drug and Alcohol Project

Screening Tool

Not to be used as a questionnaire

If you are concerned a child is in danger or there are Child Protection issues, refer to your Child Protection Policy.

Although many young people will try drugs/alcohol at some time, most do not progress beyond experimentation.

This screening tool is designed to help if you have concerns about a young person regarding their drugs/alcohol use. The main purpose is to identify appropriate interventions and indicate when specialist advice should be sought.

If a referral has been identified from the screening tool, please complete aYDAP referral form. If a priority has beenidentified please telephone us with the young person’s details beforesending the form. Completed forms should be forwardedto:

For further information on YDAP or information about completing the screening tool, consult the accompanying booklet or call 0191 5614000 to be put through to someone who can help you.

  • Facilitate a referral to YDAP
Things to consider:
  • The young person should be made aware of any referral, and should be fully involved, with their written consent given.
  • It is good practice to involve parents/carers in any discussions relating to a referral.
  • Young people and families can self-refer if preferred.
  • A copy of the completed screening tool should be sent with any referral.

Be aware, non-drug related issues may come to light as a result of this screening tool. If so, support should be sourced from relevant agencies.
