  • Proof of Residencein Gibbs High zone required:Must provide a copy of your KUB/water bill, lease agreement, or mortgage statement with your name & the address (cannot accept a phone bill, etc.)

*If parent/sstudent/s are living w/ a relative or someone else in our zone:Must provide a notarized statement from the relative/person the parent & student are living with, stating their name & listing the parent & student names to be residing with them at that address, & providing a copy of their KUB, lease agreement, or mortgage statement.

  • Proof of Birth: *Provide one of the following - Birth Certificate copy(true certificate issued by the government w/ seal – NOT mother’s copy), OR Passport issued by any nation (translated), OR Immigration Documentation, OR Decree of Adoption or other records issued by a court –
  • TN Dept. of Health Certificate of Immunization form: Must be up-to-date/current – if coming from outside of TN, must get immunization record transferred to TN form through Knox Co. Health Dept. – 865-215-5000
  • Health Physical: Upon entering Knox Co. Schools District from another school system – provide most recenthealth physical student had but must have a TN health physical within 30 days of enrolling/entering Knox Co. Schools.
  • Proof of Custody (if applicable): MUST provide final court docs.signed by judge –ONLYthe appointed primary parent or legal guardian may register a student. *Cannot accept a Power of Attorney or notarized document - must be true & final court documents signed by a judge.
  • Completion of enrollment paperwork folder: For Gibbs High School/Knox Co. Schools
  • Driver’s licensecopy: Copy of enrolling primary parent/legal guardian’s DLfor ID purposes.
  • Official withdrawal form from student’s last school: *helpful – shows exit grades, etc.

Out-of-Zone Transfers: *School zones may be found at kgis.org

If you have already applied for a transfer and receive an approved transfer letter from Knox County Schools, you must mail in the bottom portion to let KCS know whether or not you are accepting the transfer. At that time, you will need to bring the letter of approved transfer & come in to complete the registration process with Gibbs High School.
