Submission form F – RATED CRITERIA

Proponents shall provide information required in this submission form as part of Stage II evaluation of rated criteria. Stage II will consist of an evaluation of the rated criteria to determine the high score based on the following criteria. Below is an overview of the categories and weighting for the rated criteria of the RFP. Proponents shall refer to section 3.3 - rated criteria of main document for more information. Proponents who do not meet a minimum threshold of 70 points will not proceed to Stage III of the evaluation process.

Proponents must achieve a score of at least 70% (21 out of 30) on Project Experience before a proponent will be considered for the remaining Rated Criteria. Once a proponent successfully meets this threshold, a proponent must then meet the 70% minimum threshold out of 100 to proceed to Stage III of the evaluation process. Proponents who do not score the minimum thresholds will not be considered further.

1. Project Experience - 30 points

1.1 Submission will be evaluated based on providing three (3) examples of full interior renovations or new construction of multi-disciplinary projects in occupied residential buildings completed by your company in the last five years. It must include rehabilitation work, structural, mechanical, electrical and interior finishing components, done on stand-alone homes or buildings.

Construction budget for each job shall be upto $1,000,000.00.

TCHC reserves the right to contact the references to verify the information provided and if information is substantially different than indicated the corresponding score will be reduced or zero (total 30 points, 10 points per example project).

2.0Resumes: Site Superintendent(s) and Project Manager(s)-30 points

Submissions will be evaluated based on qualification and experience of key personnel identified in the project.

2.1 Proponents must provide resumes of the Site Superintendent(s) and Project Manager. Proposed for this project. Any other submitted resumes will not be reviewed or considered.

Each resume must highlight the following information:

1.Previous and current role(s) and responsibilities;

2.Description and value of previous and current projects;

3.Qualifications and education (please list any certifications);

4.Experience in similar projects (e.g. fully occupied buildings);

5.Total years of experience

Maximum two page for each resume. (Total of 30 points or 15 points per resume)

3.Scheduling - 30 Points

Submissions will be evaluated based on the Proponent’s understanding of the project, details provided in the schedule, start and end dates, and timely completion as per the RFP schedule.

3.1 For the purpose of the evaluation, provide specific work plan and scheduling as specified in the RFP.

Provide detailed construction schedule for this project in a Gantt chart format. (5points for each item below).

Gantt chart must include:

1)Start date

2)End date

3) Key activities

4)Construction phases

5) Substantial completion

6) Total completion

Do not provide generic or sample. Please provide project specific details.

4.Health & Safety – 10 points

4.1Workplace Injury Summary Report (WISR) (5 points)

Proponents shall include WSIB’s Workplace Injury Summary Report (WISR) with the proposals. WISR shall show the record for 2013, 2014, 2015, 2016 and year to date for 2017.

4.2Please provide a signed corporate mandate (Policy Statement) to safety which details that the company will provide a healthy and safe workplace, meeting all government regulations. Policy Statement must be dated (a year or less) and signed by an authorized company representative. (5 points)
