REGISTRATION REQUIREMENTS
Children attending Komarek School MUST reside within the district boundaries.
Every parent/guardian, new and returning students, must present proofs of residency.
Please note, only a parent or guardian who is a legal resident of District 94 can register a child for Komarek School.
NEW STUDENTS: PRE-SCHOOL, KINDERGARTEN, and GRADES 1 – 8: Please be prepared with the following.
- Home owners: A copy of the latest property tax bill on the residence, plus three other forms of identification with the home address on it. This can include: electric bill, gas bill, cable bill, auto or home insurance, or bank statement.
- Students living in an apartment: Parents/Guardian must provide a current and valid lease for the apartment along with three other forms of identification stated in “A” above. The district will contact building landlords to verify residency.
- Parents must present an original copy of the child’s birth certificate.
- Parents must have completed health and dental forms (These are due on or before the first day of student attendance. Students cannot begin school without health/dental forms on file at school.)
- Grades 1 – 8: Parents must present an Illinois Transfer Form from the previous public school and any records including report cards, test scores, etc. Students transferring from private school must present the final report card from the
Various school and sport forms will be available, before you sign any form, please read it carefully. We will be happy to explain any item at registration. By signing these forms, you and/or your child are responsible for all the conditions outlined in each. Please let Mrs. Anderson know if you will need the assistance of an interpreter.
BOOK FEES FOR 2016 – 2017
The District 94 Board of Education will not be increasing Book Fees for the 2016– 2017school year. They are:
BOOK FEE:Kindergarten to Grade 8
$107.00 for child #1 in family
$70 for child #2 in family
$36.00 for child #3 in family
No Book Fee for child #4 and beyond
TECHNOLOGY FEE: 5th, 6th, and 7thgrades Only: $140.00 per student (Please note, $40.00 of the Technology Fee cannot be waived.
APPLIED ARTS FEE:Grade 6 – 8 grades only: $45.00 per child
This fee covers materials for Industrial Arts and Home Economics Classes
MUSICAL RECORDER FEE: 3rd and 4th grades only:$7.00 per child
ATHLETIC FEE: The Athletic Fee is for students in Grades 6 – 8 who wish to participate in one or more of the Interscholastic Teams at Komarek. The teams are: Fall Sports, Girls Volleyball and Soccer (co-ed); Winter Sports, Girls Basketball, Boys Basketball, and Cheerleading; Spring Sport, Boys Volleyball. A limited number of students are selected for these activities after a “try out” session. Students who are not selected for any activity over the course of the school year will receive a refund of the Athletic Fee.

$25.00 for all additional children in family
(Children must have one common parent)
Any child who participates in an interscholastic sport must have a valid sports physical. This physical must be less than one-year old at the conclusion of the activity
MANDATORY GYM UNIFORM: Grades 6 – 8 only:
Students in grades 6, 7, and 8 must have a Komarek gym uniform consisting of a tee shirt, shorts, sweat shirt, and sweat pants. You can order these at the time of registration.
The cost for the gym uniforms is: Tee shirt: $12.00
Shorts:$16.00 or $25.00 for set
WAIVER OF FEES:
