1.  PROJECT INFORMATION:

2.  DESCRIPTION OF PROJECT:

A.  PROJECT BACKGROUND: East Douglass Drive and West Douglass Drive were recently turned over to the CE Services for maintenance. These two roads have not been maintained. The degradation is so severe that it can create a safety hazard for motorists, pedestrians, and school children attending Douglass Valley elementary. This project will include pavement replacement and upgrading pedestrian ADA ramps where necessary. Fifteen asphalt cores were taken within the project limits and ranged between 4 to 6-inches in thickness.

B.  CONSTRUCTION SCOPE: The contractor shall execute the project as shown in this SOW, additional project drawings, and general SABER Specifications.

3.  GENERAL PROVISIONS:

A.  The contractor shall supply all labor, materials, transportation, equipment, and supervision to provide a complete, finished, functional product meeting USAFA Specifications.

B.  The contractor must comply with specification sections 01 35 00 USAFA Environmental Standards and 01 35 10 USAFA Erosion Control Revegetation and Tree Care Standards which outlines provisions to help ensure contractor compliance with environmental law. Any areas disturbed by the construction shall be repaired using at least 3 inches of imported topsoil (satisfying the Standards), seeding (rate and mixture satisfying the Standards) and any other temporary BMPs until stabilization is established, where shown on the plans, blanketed (satisfying the Standards). Include a Spill Prevention and Response Plan with every project.

C.  Include in the proposal a performance period duration including on site construction time. An acceptable period of performance for this project will be between 86 and 116 days.

D.  Pre-Construction Site Survey: The contractor, prior to the start of any on site construction activities, shall conduct a condition survey with the construction inspector, after which the contractor shall note the condition of trees, shrubs, grassed areas, concrete, and any structural damage that may exist immediately within or adjacent to work site.

E.  The work area will be confined to the applicable areas as shown in Attachment A and as listed in this SOW. All work must conform to the provisions of the USAFA Architectural and Engineering Services Guide available on CD from the Project Manager.

F.  Submittal Register: A submittal register is provided for this project, and is a supplemental document to the SABER Specifications. Provide submittals and maintain the Submittal Register throughout project construction period of performance, as required by SABER Specifications Section 01 33 00, Submittal Procedures. This list of submittals is not all-inclusive; other submittals and forms may be required by the current Front Range SABER Contract documents. Contractor shall submit the Master Submittal Register to Contracting within 14 days after the Notice to Proceed (NTP).

G.  All hazardous construction materials must be approved by USAFA prior to use.

H.  The Front Range SABER coefficient for this project is Standard/Non-secured more than $750K.

I.  For all projects that have a Period of Performance that is 90-days or longer in length, The Contractor shall prepare and submit a DRAFT DD Form 1354, Transfer and Acceptance of Military Real Property, for all installed property (by building number if multiple buildings are involved in the project/task order) to the CO. The DRAFT 1354 shall be submitted no later than 30-days after the Notice to Proceed. This DRAFT 1354 will be reviewed by Real Property to resolve any issues and provide clarity to the Contractor for inclusion in the FINAL DD form 1354.

4.  SPECIFIC REQUIREMENTS:

A.  Traffic control will be required and must conform to MUTCD. One lane must be kept open to traffic at all times during construction hours. Two-way traffic will be allowed during non-construction hours; traffic can be allowed on base course. A traffic control plan must be submitted for approval prior to starting any demolition.

B.  The general work scope for each site is as described below and in the project design drawings. A separate proposal for each area is required:

1.  CLIN 0001 – Reconstruction of East and West Douglass Drives – The project limits, which includes the reconstruction of both East and West Douglass Drives, are shown in Attachment A (this includes shoulders and bus turnouts). The contractor shall survey the existing roadway surface to establish the design grades for the finished roadway. The limits of the intersections shall be at the point of curvature from the intersecting roads. The width of the finished road shall be the same as the current road. The contractor shall remove the existing asphalt surface layer, base course and a sufficient amount of the subgrade, to allow introduction of new asphalt and base course. The excavated material shall be disposed of off-site. The contractor shall rebuild the road using 4-in of CDOT Grading S with PG64-22 over 4-in of Class 6 aggregate base course. The existing gravel shoulder shall be maintained and re-graded to the top of asphalt elevation at the end of construction. Positive drainage should be maintained at all times, both during and after construction. Material submittals from geotechnical materials quality control testing will be required. The finished roadway shall tie into any existing pavement and concrete without noticeable roughness and to allow flow of surface drainage. The roadway shall be restriped per MUTCD Standards. Asphalt design, installation, and testing requirements to follow City of Colorado Springs Pikes Peak Regional specifications (Attachment B). These components and other miscellaneous improvements are shown in Attachment A. This work has been determined to be EEIC 52204.

2.  CLIN 0002 – ADA Accessible ramps - This work includes the construction of a total of 14 handicapped accessible ramps and paint crosswalks per MUTCD at various locations. Ramps and detectable pavers shall be installed using City of Colorado Springs details and specifications (Attachment C). These components and other miscellaneous improvements are shown in Attachment D. This work has been determined to be EEIC 52404.

C.  Utility locations will be required and noted on the as built plans.

D.  The contractor shall protect the existing curb, gutter and other surface features during the construction process.

E.  All disturbed areas shall be restored following the construction plans provided and the USAFA Erosion Control, Revegetation and Tree Care Standards. For this requirement, the contractor shall introduce a minimum of 3 inches of imported topsoil over all disturbed areas following the USAFA standards.

F.  After the work is completed, the ground surface shall be restored following the USAFA Erosion Control Revegetation and Tree Care Standards and the construction plans provided.

G.  Coordinate work schedules with 10 CONS and the USAFA Construction Manager at least one (1) week in advance.

H.  A CLIN Schedule is included. This CLIN Schedule summarizes the total costs for the work. The contractor shall additionally provide detailed cost breakdowns for the total costs listed on the CLIN Schedule. The detailed costs shall be completed using the e-4Clicks software (standard, non-secured, more than $750k). The CLIN Schedule and the e-4Clicks CLIN proposal sheets will be used by the Government for purposes of evaluation of proposals. However, award of a task/delivery order will be on a firm-fixed-price, lump-sum basis for the final negotiated total price.

I.  Maintain redlined as-built drawings in the project construction office (or trailer) during the duration of the construction. The redlined drawings must be available for inspection during normal working hours.

J.  Any exceptions to this Statement of Work must be detailed in the contractor proposal.

K.  Submittals: Contractor to provide submittals as listed in the Submittal Register.

L.  Submittals will be expedited to the fullest extent possible.

M.  Any exceptions to this statement of work must be detailed in the contractor proposal.

5.  PROJECT CLOSEOUT:

A.  Project Closeout Deliverables – Notify the Real Property Office of the construction closeout via the Contractor completed 1354 form (Note: Beneficial Occupancy Determination (BOD) must be completed prior to submitting Real Property Form 1354 to the Real Property Office). All drawings, O&M Manuals, and CDs shall be delivered to the Construction Representative within two (2) weeks of the project closeout.
