ORDER FOR INDIGENT DEFENDANT TO OFFSET COST OF LEGAL SERVICES PROVIDED BY THIS COUNTY DURING PENDENCY OF CHARGE AND PLACING ON PRETRIAL SUPERVISION
WHEREAS, the Court previously determined that the Defendant is indigent under the procedures and standards adopted by the judges of this county and has appointed counsel to represent the defendant in this matter, the Court now FINDS that:
Determination 1: The cost of the legal services provided to the defendant as of this date is
$______. This amount is reasonable and based on the itemized bills in the record, this county’s flat fee schedule, or the Court’s calculation in the record of the cost of legal services provided to the defendant by a public defender or contract defender.

3.Comply with all appointments set by your attorney. This includes any appointment set for the completion of a Pre-Sentence Investigation.
4.Report any change in residence, phone number or employment to your supervision officer.
5.Do not violate federal, state or local laws.
6.Contact your supervision officer within 72 hours of any police contact.
7.Submit to being fingerprinted by the supervision officer, if prints are needed.
8.Report monthly to the Rusk County District Supervision Officer in person or by mail.

By signing this order, the defendant agrees to abide by the conditions and understands that their performance under this order will be reported to the Court and could impact the decision in this case. The defendant also releases Rusk County and the Rusk County District Court Supervision Office from any and all liabilities under law in regards to contact with references listed on the defendant’s data sheet.
