SBI 2 training attendees have been added with editing privileges. You will be able to see pages that are shared across the state (Announcements, Discussion, Resources) as well as a page that is visible only to participants from your program/region and VALRC facilitators.

To add to or edit site pages, scroll to the top of the screen and click on the Edit icon. You will then be able to type on the page. A toolbar menu with helpful functions will appear near the top of the screen. When you have finished editing, make sure to scroll to the top of the page and hit the Save button.

To create a new page, click the Add page button at the top of the screen. You will have the choice of adding the page to the main site (“top level,” visible to everybody) or nesting the page under your region’s or program’s section of the site.

By September 15, 2016, each region or program should complete and submit the PLC Plan form. It can be found online at:

Someone who attended the PLC should complete the PLC update form after each session.

The update form is designed to be completed quickly but capture important information about the session. Questions 1-6 ask for basic demographic and attendance information, while Questions 7-11 record successes and challenges.

  11. *How many times has your PLC met? (Select "1" if you are reporting on your first meeting, "2" if reporting on your second meeting, etc.)
  12. *Provide a brief description of the PLC meeting.
    Provide at least a few sentences describing how the particular PLC session went – what activity was discussed, how participants responded, any particular insights made while following the protocol steps, what participants took away from the activity redesign.
  13. How did participants apply lessons learned from the last PLC meeting? Briefly summarize what was shared at the start of the meeting (unless this was the group's first meeting). Although the form does not require, please be sure to fill out this section showing the impact of PLCs.
  14. Please describe any questions or concerns that came up during or as a result of this PLC meeting.
  15. Would you like some support or technical assistance from the Resource Center as a follow-up to this PLC meeting? Request a follow-up phone call, virtual site visit, or site visit and include contact information for follow-up.
