2.  Explains purpose and structure of encounter
·  EXAMPLE: ‘I’m here to speak with you about your medication history. It’s helpful to have an accurate record of your experience with medications to make sure you are on the safest and most effective medications for you.’
3.  Obtains permission for encounter
4.  Uses appropriate questions
·  Open-ended questions to initiate a line of inquiry – EXAMPLE: ‘What prescription medications you are currently taking?’
·  Closed-ended/targeted questions for follow up – EXAMPLE: ‘Do you regularly drink beverages that contain caffeine?’
·  Does not use leading questions – EXAMPLE: ‘You take your medication as directed, right?’
5.  Closes encounter appropriately
·  Thanks the patient for their time
·  What will be done with information gathered
·  Asks for and answers any last minute questions
6.  Organizes the interview
·  Logical flow to questioning
·  Does not skip around
7.  Uses appropriate terminology
·  Uses terms easily understood by patient
·  Does not use excessive medical jargon
8.  Was respectful
·  Engaged in encounter
·  Does not interrupt patient excessively, no use of sarcasm
9.  Was confident
·  Clear and audible speech, no excessive use of non-words (e.g. um, uh)
·  No excessive use of nervous movements (repetitive adjustments, leg shaking, etc)
·  Good eye contact, open body language, relaxed facial expressions
10.  Dresses professionally
·  Good personal hygiene, clean white coat, close-toed shoes, no excessively short dresses or skirts, no excessive use of perfume/cologne, make-up or jewelry
·  No jeans, hats (unless they are of a religious nature), t-shirts, sweatpants, or shorts
