Standard of care review report
This form is to be used to record a standard of care review for children placed in out-of-home care. Refer to the Child Safety Practice Manual, Chapter 9, Standards of care, for the procedures for conducting standard of care reviews for care services.
To record a standard of care review:
- create a ‘Matter of Concern’ (MOC) event in the child’s profile
- add any other relevant parties to the MOC event, including the carer or care service and any relevant staff members
- complete the ‘Record of concerns’ form in the MOC event
- delete the ‘Record of checks and consultation’ form from the MOC event
- For an Aboriginal or Torres Strait Islander child, create a ‘Recognised entity participation form’ in the ‘Information gathering/decision-making’ block
- do not create any additional forms, such as the ‘Child placement concern report’
- when the ‘Standard of care review report’ is completed and approved by the team leader, attach it to the MOC event under the ‘CPCR Assessment/Action’ block
- create and complete the ‘Closure form’ in the MOC event, to close the event and enable data reporting.
