STAKEHOLDER FEEDBACK DATA DOCUMENT

Survey Administration

(Name of School) administered the Catholic Identity Defining Characteristics surveys in 20xx-xx and the Catholic Identity Program Effectiveness surveys in 20xx-xx. All surveys were administered online through a link on the Catholic Schools Office website.

Describe how the surveys were administered to each group and how you encouraged participation by each stakeholder group. Include methods of communication used such as newsletters and telephone calls. If a letter was sent home, scan it and include it at the beginning of the data document.

The Assessment for Catechesis and Religious Education (ACRE) has both a faith knowledge test and a survey component. The Diocese of Baton Rouge Catholic schools have included results of the survey portion from the ACRE as part of the survey process for the accreditation self-study.
