St. Johns CARES ASSIST Scholarship 2014-2015

(Aid and Support for Student in Sudden Transition)

TheSt. Johns CARESASSIST Scholarship will beawarded through an application process to College(s) of choice and requires approval of the Scholarship Committee. Scholarships will be limited to funds available for this program.

The scholarship will be issued for the program requested on the application and the award may only be used for this program.

The scholarships are awarded for twelve (12) months and may be used for academic expenses. Applications for the 2015-2016 school year must be submitted to the college no later than April1,2015. Scholarship recipients will be notified of the scholarship awards within three to five weeks. The St. Johns CARESASSIST Scholarship award is contingent on the successful attainment ofall requirementsfor high school graduation. Scholarship applicants must meet all requirements for the program they plan to attend.

ELIGIBILITY

1. High school senior who exemplifies the Six Pillars of Character Counts and isin good academic standing without attendance or disciplinary issues.

2. Recommendation by a school official of St. Johns County School District and a member of the local community.

3. Grade point average of 2.5 or higher.

4. A Florida resident.

5. Confirmed acceptance to College of Choice.

6. Enrolled in St. Johns County ASSIST Program.

INSTRUCTIONS

1. Complete the St. Johns CARESASSIST Scholarship Application.

2. Turn in all required documents:

•Essay

•Transcript including class rank

•Two letters of recommendation

What is the St. Johns CARESASSIST Scholarship?

St. Johns CARES and the ASSIST program of St. Johns County School District,will be awardinga

$1,000.00 scholarship to a deserving senior who is enrolled in the ASSIST Program and who has been accepted into College of choice for the 2015-2016year.

Who selects the recipient?

A selection committee comprised of ASSIST leaders, St. Johns CARESadministrators and community participants will review applications and make the selection.

Who is eligible for the scholarship?

The scholarship funds are available to students enrolled in the ASSIST Program and attendinga school inSt. Johns County.

How do students apply? Applicants must submit the following:

1) A completed applicationform.

2) A500-750word essay (essay evaluation criteria listed below)

3) An official or unofficial school transcript, which includesclass ranking.

4) Two letters ofrecommendation; one must be from a school official from St. Johns County

SchoolDistrict.

Along with the letters of recommendation, the essay is the most important component of the application.

2) Explain your desire to attend college and why you are an appropriate candidate to receive this scholarship.
