61st ANNUAL CONVENTION APPAREL ORDER FORM

As part of the 61st annual convention of MICA, we are promoting MICA embossed items. The following items are available for purchase;
Sport Shirts -- The shirts are the Nike Golf – Dri-Fit pique sport shirts embossed with the “MICA” logo. This year’s shirt color is Lawn Green for both the men and ladies, with the “MICA” logo embroidered on the left chest and “Prior Lake, MN” on the right sleeve. Each registered member (not spouses) will receive a complimentary shirt (See registration form for complimentary shirt). Additional shirts may be ordered at the prices indicated below.
Microfiber Club Jacket - 100% polyester microfiber jacket. Wind and water resistant. Relaxed elastic waistband and cuffs, contrast inside collar band and under collar, button tab on collar, raglan sleeves, 2 front pockets, inside polyester sleeve lining and inside left chest pocket with Velcro closure. Stone with Navy trim, available in sizes Small - 4XL. The MICA logo and the words “Established in 1956” will be embroidered on the left chest.
Microfiber Windshirt - 100% polyester microfiber jacket. Wind and water resistant. High overlap V-neck flat knit rib collar with contrast tipping, side entry welt pockets, fully lined, raglan sleeves, comfortable elastic waistband and flat knit rib cuffs. Stone with black trim, available in sizes Small - 4XL. The MICA logo and the words “Established in 1956” will be embroidered on the left chest.

METHOD OF RESERVATIONS
The room rate during the convention dates is $199.00 per night plus applicable taxes. All guest rooms are run-of-the-house. Rates are based upon single or double occupancy. Extra individuals are $10.00 per person, per night, plus tax. Guestroom types (kings, double/doubles, etc.) cannot be guaranteed and will be reserved on a first-come, first-served basis. To ensure that event guests receive the negotiated group rate, they will need to identify themselves as participating in the Midwest Insulation Contractors Associati event. Once the reservation is made all changes, additions or cancellations should be directed to the reservations office. All reservations must have a credit card guarantee. A valid credit card is required at check in, with a minimum authorized amount of $25.00. Check in time is 3:00 PM and check out time is 11:00 AM. The minimum age to rent a room is 21 years of age and photo ID is required at check in.
PARKING
Valet parking is complimentary for Hotel and Casino guests. Self-parking is complimentary and is available on our surface lots and parking structures adjacent to the casino/hotel complex.
INTERNET
The Hotel offers complimentary wireless Internet access in all guestrooms. Complimentary wireless Internet is also available in our lobby level public spaces.
SPA DISCOUNT
A ten percent (10%) discount at the Spa at Mystic Lake is available with reservations made in advance.
BEVERAGE REGULATIONS
Outside alcoholic beverages are not permitted on Hotel property, including meeting areas, parking areas, or Hotel rooms. Minnesota state law prohibits persons under the age of 21 from purchasing or consuming alcoholic beverages. Hotel reserves the right to verify the age of any individual at any time. Persons who appear intoxicated will not be permitted to purchase or consume alcoholic beverages at the discretion of Casino Hotel staff. Drinking games and or associated equipment are not permitted. It is Group’s responsibility to communicate this policy to all attendees.
Hotel Policies: Due to the fact that Mystic Lake is a casino; kids must to be 18 to gamble and 21 to reserve their own room at the hotel. For their safety, kids need to have an adult with them at all times. They can dine and use the pool etc but they need to have an adult with them.
