As you may be aware, the new SouthGlasgowUniversityHospital and new RoyalHospital for Sick Children are due to open on the current Southern site early in 2015.

With this in mind, please note that positions based within the Victoria Infirmary, Mansionhouse Unit, the Western Infirmary and the current RoyalHospital for Sick Children at Yorkhill will change location and move to the new hospitals.

Gartnavel GeneralHospital and Glasgow Royal Infirmary will also have some services affected by moves to the new Hospitals.

Applications are invited for a Specialty Grade Doctor in Paediatric Haematology and Oncology. This whole time post is based at the RoyalHospital for Sick Children (RHSC), Glasgow, but will move to the Southern General Hospital (SGH) site when paediatric services relocate to the New Children’s Hospital in June , 2015. This is a new post with an immediate start date.

The main purpose of the post will be to undertake clinical duties. The main duty of the post holder will be to manage inpatients along with a team of middle grade trainee doctors and FY2s. The successful applicant will take part in the haematology and oncology middle grade rota as a fixed commitment. The post-holder will be expected to participate in the undergraduate, postgraduate and nurse training programmes.

The appointee will contribute to the existing team of 3 Specialty Grade Doctors, an Associate Specialist and 8 Consultants within the department.

All applicants must possess Full GMC Registration, a licence to practice and at least 4 Years full time Post Graduate Training, two of which must have been in relevant acute specialties or you will demonstrate and evidence equivalent experience and competencies.

Section 1Children’s Services across NHS Greater Glasgow & Clyde

The Royal Hospital for Sick Children (RHSC), Glasgow, is the largest paediatric teaching hospital in Scotland. It provides care, not only for children resident within Greater Glasgow, but is also a tertiary referral centre for children from the West of Scotland and, in some subspecialties, from the whole of Scotland. It has 266 inpatient beds, 12 daycase beds, and handles approximately 90,000 out-patients, 15,000 in-patients, 7,300 daycases and 35,000 A&E attendances every year. The hospital provides care for children from newborns up to around 16 years of age.

All paediatric medical and surgical subspecialties are represented, including general medical paediatrics, cardiology, neonatology, neurology, nephrology, respiratory, endocrinology, gastroenterology, immunology and infectious diseases, dermatology, haematology, oncology, rheumatology, metabolic medicine, audiology, ophthalmology, ENT surgery, orthopaedics and general paediatric and neonatal surgery. The critical care service includes a high dependency unit, an intensive care unit and a combined neonatal medical and surgical intensive care unit, as well as a paediatric and neonatal retrieval service. The hospital provides a national ECMO service and is the centre for all paediatric cardiac surgery, bone marrow transplantation and renal transplantation in Scotland. A selection of child and adolescent psychiatry facilities are located within the campus.

In its role as a major academic institution, RHSC is home to a number of University departments as well as internationally acclaimed research groups. The significant commitment to the teaching and training of new doctors, nurses, midwives and other health professionals ensures that highly trained NHS staff are ready to care for the children of tomorrow. It provides the major undergraduate paediatric teaching facility for the University of Glasgow.

The diagnostic imaging department located within RHSC provides ultrasound, CT, MRI and isotopic studies on site. Diagnostic laboratory facilities on site include haematology, blood bank, biochemistry, microbiology, virology, histopathology and genetics. Specialist laboratory services have re-located to the Southern General Hospital (SGH) site, but appropriate laboratory services will remain at the RHSC site until the hospital relocates in 2015.

Other Paediatric Services in NHS GG&C

There are currently three maternity hospitals in Glasgow/Paisley; Princess Royal Maternity Hospital (PRMH), Southern General Hospital (SGH) and the Royal Alexandra Hospital (RAH), all with neonatal facilities.

Paediatric neurosurgery has historically been located in the Institute of Neurosciences at SGH, but was relocated to the RHSC site two years ago.

There is one children’s ward at the RAH providing local care for the children from Paisley.

There is an extensive range of specialist community based children’s services across NHS Greater Glasgow. Managed within community health and social care partnerships, these services are integrated with primary care and social care services.

Section 2New Children’s Hospital

A new children’s hospital has been built on the Southern General Hospital (SGH) site and is co-located with a new adult hospital and a re-developed maternity hospital. The predicted opening date for the new RHSC is mid-June , 2015. The adult stem cell transplant unit will also relocate to this site facilitating a closer relationship. Cytogenetics and diagnostic molecular haematology and oncology are located on this site.

Section 3Paediatric Haematology and Oncology Services

The clinical Haematology / Oncology service is part of the Women and Children’s Directorate of NHS GG&C providing care for children with leukaemia, solid tumours, brain tumours, bleeding disorders, haemoglobinopathies and a range of benign haematological conditions from the West of Scotland. It houses the National Paediatric Haemopoietic Stem Cell Transplant (HSCT) Unit and is the Regional Paediatric Haemophilia Centre. The department is a Children Cancer and Leukaemia Group (CCLG) Centre and eligible patients are entered into appropriate NCRI protocols.

In-patient Facilities

There is a dedicated Haematology / Oncology Unit (Schiehallion Ward) with 22 inpatient beds, of which 16 have resident parent/carer facilities and 2 HEPA-filtered rooms for HSCT. There is a comprehensive Day-care facility with consulting rooms and 7 day-care beds/reclining chairs. There is a small suite within the Unit for resident parents and additional parent accommodation in the adjacent CLIC-Sargent supported Cruachan House or Ronald McDonald House. There is a small classroom with provision for both primary and secondary education and generous play room supported by play therapists. There is a pharmacy suite within the unit with dedicated oncology pharmacists for the preparation of chemotherapy. The electronic chemotherapy prescribing system (Chemocare) is in use and the uploading of protocols is ongoing .The Unit is JACIE and HTA accredited. The unit is actively recruiting eligible patients for NCRI and commercial trials and is a recognised ITCC centre.

A 6 bed adolescent unit funded by the Teenager Cancer Trust and a refurbished day care facility with expanded facilities opened in 2010 on the seventh floor of RHSC.

Patient facilities will be replicated in the New Children’s Hospital.

Middle grade staff do not participate in the general medical paediatric on-call rota, but provide dedicated out-of-hours cover to the Haematology / Oncology Unit up to 21.30hrs including the weekends. Overnight cover is provided by the hospital at night team.

National Arrangements - Shared Care

Stem Cell Transplant is supported by the National Service Division (NSD). Paediatric Oncology and Malignant Haematology is now a national service supported by the Managed Service Network (MSN) for Children & Young People with Cancer. Scotland has two level 4, one level 3 and one level 2 treatment centres. Shared care arrangements currently exist between the RHSC Glasgow and local District General hospitals in Dumfries and Inverness. Within this arrangement, in-patient based chemotherapy and intensive aspects of chemotherapy are delivered at RHSC Glasgow with outpatient reviews and supportive care arranged locally. Shared care clinics are held every 3 months for haematology and every 6 months for oncology in Inverness. Shared care units provide support, including certain investigations (blood counts) and treatment (febrile neutropenia) nearer to the child's home and have an important role in the delivery of palliative care.

There are designated Consultant Paediatricians in these centres.

Existing shared care arrangements are under review.

National MDTs for leukaemia take place weekly and solid tumour MDTs fortnightly. National palliative care MDT takes place every two weeks either by tele or videoconference. A national blood transfusion policy and a national guideline for febrile neutropenia are in place and there is a national educational programme for trainees which is conducted by V/C.

Section 4Duties of the Post

The successful applicant will have responsibilities in all areas of clinical care, including ward, day-care and outpatients. However the main responsibility lies in the day to day management of inpatients on Schiehallion ward and TCT unit. The post holder, after appropriate training will also undertake responsibilities such as chemotherapy prescription, LP, IT administration, bone marrow aspiration and trephine biopsy. He/she should have clinical paediatric experience. He or she is also expected to work fairly independently but with supervision, support and advice is available at all times from seniors. He/she will be responsible for the administrative duties associated with the care of patients and will participate in the on-call service.

Out of hours work will comprise an evening session 1 in 6 (17:00 - 22:00hrs) and one weekend in 6 (Friday 17:00 – 22:00hrs and Saturday/Sunday 09:00 – 22:00hrs). No overnight cover is expected from this post either during the weekdays or weekends.

Research & Development

The successful candidate will be expected to maintain personal professional development appropriate to the post. There will be opportunities for individual or collaborative research and development within the service.

Teaching

The successful candidate will be expected to engage in undergraduate and postgraduate teaching, appropriate to experience.

Proposed Job Plan

The post holder will be one of 4 Staff Grade Doctors who will provide continuity of care across all aspects of the service.

The following work plan can be adjusted as needed, depending on personal and departmental needs.

The contract is for up to 10 Programmed Activities.

These 10 sessions would consist of:

The actual timing of these sessions is negotiable, but it should be reiterated that there must be considerable flexibility.

Facilities and Education

A number of educational hospital meetings occur regularly, including an afternoon dedicated to continuing education which is accredited for CME purposes. There is a weekly departmental educational meeting which takes the form of case presentations/journal club.

General

All medical and dental staff employed by the Acute Services Division are expected to comply with all Hospital Health and Safety Policies applicable on the various hospital sites.

The successful candidate will be expected to organise his/her workload and operate an appointment system in a manner that is consistent with good practice.

The post-holder will be expected to undertake the administrative duties necessary to the tasks listed above, to the care of his/her patients.

The post-holder will be expected to establish and participate in audit projects involving the service delivery of the departments and be aware of clinical governance and clinical risk management.

The post-holder will be expected to contribute to the management of the department at a level commensurate with experience and to co-operate with operational policies of the department and directorate.

Managerial

The management responsibility of the post-holder will be to the Link Clinician for Haematology and Oncology Services, Women’s and Children’s Directorate (current post holder is Prof Brenda Gibson).

The applicant must have full registration with the General Medical Council and produce evidence of this prior to commencing the role.

The applicant should have completed at least four year’s full-time postgraduate training (or it’s equivalent gained on a part-time or flexible training basis) at least two of which will be in paediatrics .or paediatric haematology /oncology. Additionally, the applicant should have sufficient experience to exercise an intermediate/middle grade level of clinical responsibility delegated by the Consultant.

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.
