Applicants are invited to apply for four available sessions of Specialty Doctor in Dermatology based in North Glasgow. Dermatology services in NHS Greater Glasgow & Clyde are provided by the Dermatology Department, the Alan Lyell Centre for Dermatology. The Department works on a hub and spoke model, with in-patient beds based at the QueenElizabethUniversity Hospital (QEUH). The development of the QEUH as the hub site offers tertiary and clinical academic services as a regional dermatology centre for approximately 2.4million people in the West of Scotland. Local dermatology services are delivered at eight hospital sites across the city – Glasgow Royal Infirmary, QEUH, StobhillHospital, NewVictoriaHospital, Western Infirmary, RoyalAlexandraHospital, InverclydeRoyalHospital and the Vale of Leven Hospital. Each of these sites provide a full range of dermatology out-patient services.

The Specialty Doctor sessions are available within the following sites:

You will be a suitably qualified doctor who will assist the consultant teams with new and return outpatient clinics and minor procedure lists. Previous dermatology experience is essential and a diploma in dermatology is desirable.

All applicants must possess full GMC Registration, a licence to practice and at least 4 Years full time Post Graduate Training, two of which must have been in relevant acute specialties or you will demonstrate and evidence equivalent experience and competencies.

About the Board

Core adult acute care is currently delivered from 5 sites within Glasgow and three sites in Clyde. Gartnaval GeneralHospital delivers acute care in the west-end of the city. In the north-east of the city acute care is delivered from StobhillHospital and Glasgow Royal Infirmary. The NewVictoriaHospital serves the south-east population and the QueenElizabethUniversityHospital serves the south-west of the city. Services for children are provided centrally from The Royal Children’s Hospital based on the south west Glasgow campus Full Accident and Emergency services are provided at the Glasgow Royal Infirmary and Queen Elizabeth University Hospital. In Clyde, acute services are currently provided at RoyalAlexandraHospital, InverclydeRoyalHospital and Vale of Leven Hospital.

The Hospital Modernisation Programme will ensure that walk-in / walk-out hospital services are provided for the vast majority of patients. The pattern of service provision will shift to reflect moves towards ambulatory care. Currently 85% to 90% of patient encounters with acute hospital services are on a walk- in / walk-out same day basis. These include out patient attendances, diagnostic tests, imaging procedures, and a range of day surgery procedures. In future, these services will be provided from ambulatory care centres designed to deliver streamlined processes of care, which patients want – to be seen quickly by the appropriate specialist, to undergo clinical investigation, and to receive treatment without delay.

The ambulatory care centres for the south side and north east of the city of the city are in the NewVictoriaHospital and New Stobhill hospital respectively. These state-of-the-art facilities opened in June 2009. They house the main out-patient centre and day surgery services. In the south side in-patient services are concentrated in The Queen Elizabeth University Hospital. This new facility, housing some 850 beds, has replaced ageing acute wards in both the Southern General Hospital and the Victoria Infirmary. The new facility will work alongside some of the relatively modern buildings housing specialist services, which will be retained on the South site as part of the Strategy.

The redesign and redevelopment of Glasgow’s acute services will address many of the pressures currently facing the hospital service. The new service will be provided in modern facilities rather than 19th century buildings not designed for modern healthcare. The purpose designed facilities will enable one stop / rapid diagnosis and treatment models required for the future. Continuity of service will improve with the elimination of the need for patient’s notes and results to be moved from building to building. Concentration of services will allow the requirements of junior doctors’ hours and issues arising for increasing sub specialisation of medicine to be addressed through the creation of larger teams and sustainable rotas for both junior and senior staff.

The formation of larger clinical teams will make sure that programmes of work, including the need to cover emergencies without interfering with waiting list and ambulatory care sessions, can be planned effectively. The concentration of in-patient services on fewer sites will help strengthen specialist services and maximise the capacity of the service.

All the above services are supported by other Consultant-based services, which incorporate diagnostic and clinical components including Anaesthetics, Laboratory Medicine, Neuroanaesthetics, Neuropathology, Clinical Neurophysiology (including EEG, EMG and EP), Neuroradiology and Radiology including Spiral CT Scanner. There is also available a wide range of therapeutic services which include Audiology, Clinical Psychology, Dietetics, Occupational Therapy, ECG, Physiotherapy and Speech and Language Therapy.

There are close links with the Mental Health and Elderly Mental Health services provided by NHS Greater Glasgow & Clyde Community Health Partnerships.

Acute Services Division

NHS Greater Glasgow & Clyde is the largest Health Board in Scotland. The Board provides a comprehensive range of services from community based care (midwifes, dental services and various outreach services) through to a full range of general and specialist hospital services, including regional and national specialist services. In addition, the Division has close links with the universities in Glasgow and makes a significant contribution to teaching.

The Division has a catchment population of approximately 900,000.*

Dermatology services within Glasgow & Clyde are currently provided from eight main hospital sites – Glasgow Royal Infirmary, InverclydeRoyalHospital, RoyalAlexandraHospital, QEUH, StobhillAmbulatoryCareHospital, Vale of Leven Hospital, NewVictoriaHospital and Western Infirmary.

Glasgow Royal Infirmary

Situated on Castle Street, Glasgow Royal Infirmary - or GRI - has 1077 beds and provides broad range of regional, supra-regional and national acute clinical services. The Division recently completed a £60m capital development programme at the GRI site. This saw the creation of major new developments which have significantly improved healthcare facilities for the people of North Glasgow. Further to the closure of CanniesburnHospital, the GRI now has a new Plastic Surgery and Burns Unit, in addition to its new Emergency Receiving Centre, based within the JubileeBuilding.

Princess Royal Maternity

PrincessRoyalMaternityHospital, which is adjacent to Glasgow Royal Infirmary on Alexandra Parade, has 140 beds and provides maternity and special care baby unit/ITU facilities for neonates.

The Directorate of Dermatology

Dermatology services provided in the adult acute sector of NHS Greater Glasgow & Clyde underwent major redesign culminating in the development of a ‘hub and spoke’ model of service delivery. Out-patient and ambulatory treatment services continue to be provided locally on the following sites:

In January 2005, all Glasgow adult inpatient beds were consolidated at the QueenElizabethUniversityHospital as part of the creation of the dermatology hub. The development of the QEUH as the ‘hub’ site offers tertiary and clinical academic services as a regional dermatology centre for approximately 1.5 million people in the West of Scotland. This consolidation of inpatient beds was completed in June 2008 when the inpatient beds at the RAH were centralised at the Southern site.

The unified hub and spoke service was officially launched in 2006 as the Alan Lyell Centre for Dermatology.

The Work of the Department

Dermatology services are presently based at, QEUH, Glasgow Royal Infirmary, StobhillHospital, the Western Infirmary and the Victoria Infirmary. Paediatric dermatology services are provided at The Royal Children’s Hospital.

Western Infirmary

The Western Infirmary provides a full range of out-patient dermatological services.

These services include phototherapy, day treatment and dressings, specialist psoriasis management, skin cancer screening clinics, and a clinic for renal transplant surveillance. Surgical and biopsy sessions are also undertaken at the Western Infirmary.

Glasgow Royal Infirmary

Glasgow Royal Infirmary provides a range of dermatological services on an out patient basis only. Surgical and Biopsy lists are also undertaken on this site. Phototherapy and Day treatment services are also provided from Glasgow Royal Infirmary.

StobhillAmbulatoryCareHospital

Dermatology within the new StobhillAmbulatoryCareHospital provides a range of out patient based dermatology services. These include general out patient clinics, phototherapy, photodynamic therapy, day treatments, biopsy sessions and surgery.

QueenElizabethUniversityHospital

The QEUH will be the main in patient ‘hub’ unit for Dermatology provision. There are 12 in patient beds serving Glasgow and the surrounding population. Day treatment, Phototherapy and Photodynamic therapy are provided from the integrated complex at ward 2A.

Out patient services are currently provided from a dedicated out patient dermatology unit situated alongside 2A. The Outpatient unit is an integrated hub and has become a centre for local and regional tertiary services which includes the Contact Dermatitis Unit, training and clinical academic dermatology.

In addition to the general out patient clinics, MOHS surgery, Hair clinic, podiatry combined clinics, genetic and Research clinics are also held within the department.

Vale of LevenHospital

Dermatology services at the vale of Leven are provided by a multidisciplinary team of practioners including a consultant, an Associate Specialist, a hospital practioner and specialist dermatology nurses, working within a dedicated Dermatology OP Treatment Centre. This provides nurse led phototherapy, patch testing and Roaccutane follow-up services. A regular Red Cross Camouflage clinic is held within the Department.

InverclydeRoyalHospital

The Dermatology Ambulatory Unit is situated with the centre ward of Level G in Inverclyde Royal Hospital and provides the following services: Consultant led out patient clinics, Nurse led clinics – Cryotherapy, Phototherapy, Chronic Disease Management, Patch Testing, Drug Monitoring, Minor Surgery, Photodynamic Therapy. Skin biopsy/Cutaneous Surgery Sessions. Outpatient treatment service, Phototherapy Service, Joint Podiatry/Dermatology Nurse Specialist clinic, Medical ward referrals, Referral from community nursing services for advice from Dermatology Nurse Specialist.

RoyalAlexandraHospital

Opened in July 2008, the new Ambulatory Dermatology Centre within the RoyalAlexandraHospital provides the following services: Consultant and nurse led treatment service, phototherapy service, outpatient treatment service, skin biopsy/cutaneous surgery clinics, nurse led treatments, medical led cutaneous surgery/biopsy lists and nurse led biopsy lists. In addition to the general outpatient clinics the dermatology services also provide – consultant and nurse led outpatient clinics, pigmented lesion clinics, cryotherapy and Chronic Disease Management by Specialist Nurse and patch testing.

New VictoriaHospital

In line with the Hospital Modernisation Programme, the NewVictoriaHospital opened in June 2009. Dermatology out patient clinics are provided from Clinic G within the NewVictoriaHospital. In addition, minor surgical procedures are undertaken as part of the out patient caseload.

The Consultant on call is supported by a ‘first on call rota’ made up from Specialist Registrars. There are no out of hours on call commitments associated with this post.

Duties of the Post

The postholder will support the existing team of medical staff within the Dermatology Directorate of NHS Greater Glasgow and Clyde. The precise nature of specialist interest and degree of responsibility within the department will be determined by the ability and enthusiasm of the successful candidate.

The arrangements for your duties will be agreed between the successful candidate and Clinical Director. The Consultant responsible for this post will be the Clinical Director, who may subsequently nominate another consultant to whom you will be responsible. The Clinical Director or nominated consultant has the right, subject to your agreement, to vary the roster of your contracted sessions and place of duty. Agreement may involve the candidate to work in one or more sites across the city.

The successful candidate will be expected to undertake General Outpatient clinics, and participate in biopsy / surgical sessions as agreed by the Clinical Director/nominated consultant.

So far as is consistent with the proper discharge of the above duties, the post holder undertakes to deputise, from time to time, for absent colleagues as agreed by the Clinical Director.

The post holder undertakes, exceptionally, to perform additional duties in the occasion of emergency and unforeseen circumstances.

Administration / Continuing Medical Education

For full time staff, one SPA session, on a Thursday afternoon, will be allocated to continuing medical education time. One additional session will also be given for administration purposes. This allocation will be on a pro rata basis for part time candidates.

All hospital sites have excellent library facilities. In addition, each of the dermatology departments hold an impressive selection of journals, educational text books, and research materials. Internet access is available via the hospital network from your P.C. Thursday afternoons are reserved for continuing medical education purposes.

Although this is not a training post, the post holder will be encouraged to attend postgraduate conferences and participate in the continuing development of the department including audit and clinical governance. The post holder will be encouraged to develop a specialist interest, in consultation with colleagues.

There is no initial expectation for the post holder to undertake research; however, this can be developed in agreement with the Clinical Director.

The post holder will have an annual appraisal and annual job plan review.

The post holder will undertake CPD and CME in accordance with their own personal development needs.

Secretarial support will be provided from within the existing pooled resources at each hospital site.
