Definition

The ILP FYI Case Manager is responsible for providing information and referrals, case management, outreach and direct services to youth and families referred to the Children’s Cabinet. This position will report to the Independent Living Program Coordinator.

Duties

  • Provides case management services to youth who are in foster care and/or have aged out of foster care ranging in age from 15-21.
  • Profile and screenfor needs of ILP clients and provide appropriate referrals to community resources.
  • Conducts weekly contacts with clients.
  • Conducts outreach at specified sites to identify youth and families in need of services.
  • Conducts client intakes for ILP Department programs.
  • Coordinates the referral process and intake process for the program including information and referral for self referred/walk-in, WCSD, WCJS, WCSS, and other community referred clients.
  • Supervises ILP interns and volunteers.
  • Initiate, complete, and maintain all required paperwork within designated timelines.
  • Enters updates and monitors information in client databases for case management.
  • Submits monthly reports as directed.
  • Manages program budget.
  • May be required to transport clients
  • Read, understand and follow FYI Policies and Procedures.
  • Achieve an understanding of agency mission, strategic focus areas and overall goals and objectives. Contribute to the success of these areas.
  • Perform other duties as assigned.

  • Keep appointments punctually, be organized and demonstrate follow through.
  • Learn new ideas and skills by reading and with hands-on-training.
  • Express ideas clearly and concisely both orally and in writing.
  • Work with adults and youth of diverse backgrounds and beliefs.
  • Problem solve in stressful situations

Special Requirements

  • Bachelor Degree or equivalent experience in a related field.
  • Bilingual preferred.
  • Able to pass complete background check.
  • Must have reliable transportation to get to and from work
  • Must posses a valid Nevada Driver’s license; and proof of insurance if utilizing personal vehicle for company business.
  • Willing to obtain needed medical screens and immunizations such at TB test, Hepatitis, etc.
