The paper listed below has been accepted for presentation at the SouthEast SAS® Users Group 2015 Conference to be held September 27-29, 2015 in Savannah, Georgia. As a speaker, you are expected to submit the paper on which your presentation is based for publication in the Proceedings. We ask that you and all coauthors sign this form and scan it and upload it to the system or email it to on or before June 29, 2015.
Only one form is to be returned to SESUG, signed by each author (except SAS authors). Do not send separate forms for each author of multi-author papers – send only one original signed by all non-SAS authors.
Your paper will not be processed and included in the Proceedings unless this form is completed, signed by all authors, and received by the publisher.
