CHILDREN WITH DISABILITIES –FINANCIAL ASSESSEMENT FORM

The purpose of this form is tohelp the Council understand and assess what additional support (including financial support)families might need in order to help them care for a disabled children.

Typically, services provided to disabled children are not subject to any financial assessment, however, where a family seeksfinancial help to fund attendance at clubs or to transport a disabled child, they will be subject to a basic assessment in order to determine if they have the means to facilitate such needs from the financial resources available to them.

The reasoning behind this, is that like other families with children, the Council typically expects families with disabled children to fund activities and transport costs, however, the Council accepts that there might be cases where families may struggle to meet these costs, we therefore may assist in exceptional circumstances, for example, if meeting these costs places the family in financial hardship and there are clear benefits associated with the activity.

In completing the form, Social Workers / Early Help Workers should use information provided by the Parent / Carer. It should be noted that completing the form will not automatically entitle a family to financial support, as the assessment process may suggest that the child’s needs could be met from existing resources available to the family.

This section is used to understand the family finances, Social Workers should endeavour to try and understand the families income and spending in order than CDT Panel can get a detailed picture of the household finances.(Wherever possible use weekly figures and obtain evidence such as a bank statement or wage slips wherever possible)
