Approx $55,000 to $64,000 per year

CTC Youth & Family Services, Murgon delivers a suite of programs which predominantly support young people and children, individuals and families connected to the Cherbourg community. Teams include Safe Haven, Wondin-dee, Youth, Family and Child Connect and Domestic Violence. Teams work collaboratively and assist each other on a day to day basis under direction of the Assistant Youth & Family Services Manager.

Fusion Target Group

Young people of the South Burnett aged 12 to 18 years who are at risk of disconnection, including young people at risk of:-

  • Disconnecting from their family/community or support network
  • Disengaging from school, training and/or employment
  • Harm, including self-harm
  • Homelessness

Purpose of Fusion Funding

To deliver a range of services that support young people who are not supported by a range of protective factors (families, friends, communities, school) that enable them to lead healthy and active lives.

Services will provide consistent assessment to identify appropriate individual responses for vulnerable young people to help them achieve positive life outcomes. These will be available for the duration of the young person’s needs, from short to longer term. Where required, services will facilitate the delivery of effective and coordinated support to young people, including those with multiple and/or complex needs.

The intensity of the support provided will be determined by the needs of the young person. The service must identify and provide the most appropriate response and/or suitable referral in accordance with the presenting level of need and circumstances of the young person.

Youth WorkerRequirements

  • Working with Children Blue card or ability to acquire
  • Ability to meet and maintain any other current and future relative legislative benchmark requirements
  • Tertiary qualifications in a human services related field or ability to acquire through an RPL process
  • Demonstrated skills in engaging young people
  • Experience in working with complex issues such as homelessness, alcohol and drug addictions, mental health
  • Ability to respond in a culturally sensitive way to service users and community need
  • Ability to uphold cultural protocols and maintain community support
  • Excellent local networks both government and non-government
  • Established networks with other organisations delivering support services to people connected to Cherbourg
  • Experience in compiling case notes and support/case plans
  • Time management and IT competency to ensure all client case notes are compiled and plansand other requirements are reviewed within the appropriate timeframes and that complete client files are kept up to date on a data capture system
  • Demonstrated understanding of personal and professional boundaries
  • Commitment to respecting the privacy and confidentiality of all stakeholders
  • Ability to maintain a manual driver’s licence without suspension
  • Current First Aid and CPR Certificates or ability to acquire

Performance Measures

  • Regular support and supervision with the Youth Services Team Leader, Murgon
  • Performance will be reviewed prior to the end of the initial 6 month probation period and annually according to the details of this and any revised position descriptions

Duty Statement as an Employee of CTC

  • Adhere to all policies and procedures of CTC and relevant Government Contracts/MOAs/ Legislation/Acts/Guidelines
  • Integrate with other programs and the ethos of CTC
  • Be an effective member of the Youth Team and other CTC teams
  • Work independently and as part of a team in compliance with direction from management
  • Attend and actively participate in all team and other relevant meetings and training
  • Assist other staff in their duties
  • Assist in maintaining a clean, hygienic and tidy work environment at all times
  • Ensure all CTC resources and assets are used only as appropriate, accounted for and maintained in a safe, clean, hygienic and useable condition at all times
  • Flexibility in regard to working hours and locations and preparedness to undertake complementary duties as required

Duty Statement as a Youth Worker (Fusion)

  • Identify and engage young people requiring assistance
  • Respond to need
  • Provide information, advice and referral
  • Undertake need assessments and provide intensive case management
  • Follow through on referrals from third parties within 48 hours or within two business days
  • Respond to self-referrals immediately (first wait time should not be more than 10 minutes)
  • Complete assessments - including CAT assessments every 3 months (practice guide available on intranet)
  • Develop client led support/case plans
  • Follow through on support/case plans at least fortnightly
  • Review support/cases plan at least monthly
  • Utilise brokerage according tosupport/case plans and within the budget
  • Ensure delivery is in accordance with the Youth Investment Specifications, YS Practice Guide and Youth Wellbeing CAT Guide (available from intranet)
  • Ensure all plans are SMART (Specific, Measurable, Achievable, Relevant and Time bound) and that these plans are followed up and reviewed quarterly to ensure that the needs of young people are met
  • Ensure support plans reflect the need for any assistance provided through brokerage funding and that any expenditure is appropriately approved, recorded and accounted for
  • Deliver and record a minimum of 28 client contact hours per week. Hours include work with or on behalf of each client
  • Ensure all case notes are entered within 24 hours of contact and that outcomes and outputs as specified in the service contract are achieved

There are three levels of engagement:

  • ACCESS - one off or short term support (no CAT or support/case plan required)
  • SUPPORT - working with a young person towards goals to overcome barriers
  • INTEGRATED SUPPORT - working with a young person with multiple or complex needs and three or more stakeholders through a single case plan

More detailed duties will be arranged with the worker’s supervisor in line with the requirements of the organisation and broadly consistent with the role, including any targets for outputs and outcomes directly related to the role.

Service standards are to be in accordance with the ethos, mission, vision, policies and procedures of CTC and in strict adherence to licencing/contractual requirements.

As part of the Youth & Family Services Team, the worker will have knowledge and understanding of other CTC services and contribute to the success of the organisation in its entirety.
