SOCIAL SERVICE ASSISTANT

Job Summary:

Assists the Social Service Director (SSD) plan and implement psychosocial programs. Aids SSD in community contacts and involvement. Assists SSD in assessing residents.

Qualifications:

  1. Be of legal working age
  2. Be able to read and follow written directions
  3. Be able to communicate with residents and co-workers in English
  4. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  5. Work overtime is necessary
  6. Must possess a general knowledge and understanding of state and federal laws as they pertain to long term care

Responsibilities:

  1. Duties
  2. Assess resident for Comprehensive Care Plans
  3. Develop, schedule and conduct psychosocial programs
  4. Chart residents need for and resident’s responses to psychosocial programs
  5. Arrange for transportation by families to outside appointments
  6. Prepare admission documentation, orientation and welcome
  7. Resident correspondence and outside contracts
  8. Any other assigned duties by the Social Service Director or Administrator
  9. Communication Skills
  10. Communicates with residents/families in a sensitive manner
  11. Communicates pertinent information and seeks assistance regarding resident care as needed.
  12. Contributes in a constructive manner in staff meetings and Resident Care Conferences
  13. Consistently communicates in a manner that demonstrates a positive and cooperative attitude
  14. Utilizes appropriate and professional body language
  15. Demonstrates receptively through active listening
  16. Consistently records on concurrent basis all observations and psychosocial needs of the residents indicated within the role of the Social Service department in the medical record and other forms as appropriate
  17. Role Involvement
  18. Performs responsibilities under the direction of the Social Service Director in a cooperative manner. Uses time constructively and organizes assignments for maximum productivity
  19. Participates in the orientation of other staff. Acts as a role model to these personnel
  20. Attends staff meetings and in-servicing
  21. Offers constructive suggestions to improve the system for delivery of resident care
  22. Requests and accepts constructive criticism regarding own professional judgment or actions
  23. Identifies and utilizes appropriate lines of authority within the organizational structure.
  24. Manages conflict and frustrations in a positive and constructive manner

  2. Demonstrates support of the philosophy of the social service department by adhering to policies, procedures and established standards of practice
  3. Demonstrates responsibility for own standards of practice
  4. Reports to work as scheduled and on time
  5. Adheres to facility policy regarding absenteeism
  6. Avoids work related injuries through the application of proper practice techniques
  7. Assumes responsibility for personal appearance through the Petersen Health Care dress code
  8. Demonstrates respect for residents rights by:
  9. Maintaining strict confidentiality of resident’s information
  10. Responding to their right to information
  11. Acting as a resident advocate
  12. Provides for their privacy
  13. Develops a rapport with peers and co-workers that is conductive to effective resident care
  14. Change Process
  15. Suggest areas appropriate for change in a manner that demonstrates a positive attitude
  16. Supports the change process in a manner that demonstrates a positive attitude
  17. Adapts to change brought about by external and internal force in a cooperative and mature way

Physical Requirements:

  1. Must be able to lift objects weighing up to 50 pounds
  2. Must be able to carry objects weighing up to 25 pounds
  3. Must be able to push or pull objects weight up to 50 pounds
  4. May be required to stoop or bend throughout the day
  5. Reaching and grasping with arms and hands, including reaching above shoulder level
  6. May be involved in stressful situations while performing job responsibilities
