Six Book Challenge 2013 - National Prize Draw Entry Form

The Reading Agency is offering four prizes for completers of the Six Book Challenge in 2013 as follows:

·  1st prize: an all expenses paid trip to London for the winner and guest. This includes two tickets to a West End show, overnight accommodation at a London hotel, travel expenses and spending money of £150.

·  Runners-up: a Kobo eReader each for three runners-up.

There are separate prize draws for participants who complete the Rugby League World Cup 2013 Six Book Challenge and for offender institutions, workplaces and colleges running the Six Book Challenge. Please make sure that all entrants for this prize draw fall within the main target audience for the scheme, ie adults who are improving their literacy skills or are just getting into reading for pleasure.

Please use the form overleaf to list entrants for the national prize draw and remember to include their contact details. This must be sent either:

TERMS AND CONDITIONS:

1. The draw is open to all residents of the UK (including the Channel Islands and the Isle of Man) and the Republic of Ireland aged 16 or over who have completed the Six Book Challenge 2013 using the standard or Welsh/English bilingual materials and who are within the main target audience for the scheme. Only one entry per person.

*Please note that a winner under the age of 18 would need to be accompanied on the trip to London by a guardian or adult.

2. All entry instructions form part of the terms and conditions.

3. The closing date for the draw is 5.00pm on Friday 28 June 2013. The winner will be drawn at random by an independent person and notified by Monday 8 July 2013 by telephone.

4. The Six Book Challenge accepts no responsibility for entries not received. Proof of sending is not proof of receipt.

5. If any winner is unable for any reason to accept a prize or cannot be contacted by Monday 29 July 2013 the Six Book Challenge reserves the right to award the prize to another participant whose name will be drawn at random from all entries received.

6. Incomplete and late entries will not be accepted. Prizes are not transferable and no cash alternative is available. No purchase necessary.

7. The judges’ decision is final. No correspondence will be entered into.

8. The Six Book Challenge reserves the right (a) to substitute alternative prizes of equivalent or greater value and (b) in exceptional circumstances to vary, amend or withdraw this competition on reasonable notice.

9. The prizewinners’ names will be announced in Six Book Challenge communications such as emails and on the website.
