Team Membership

The anchors and members of the Site Based Management team are the administrator(s), teachers, support staff, parents (ideally, parents should comprise 1/3 of team).

Optional members can include: students, community members, and co-curricular representatives. The recruitment of diverse representation is encouraged.

  1. Based upon the school size and diversity, the SBM team will be comprised of the following number of representatives from each anchor group:

Five to seven

Operational Procedures

Determine the system for how the SBM team will operate.

  1. When and how often will the team meet?

First Thursday of the month. After school, 4:00 – 5:00 p.m.

Guidance Conference Room.

  1. What specific roles/responsibilities will team members have?

Co-chair types up minutes

  1. How will meeting agendas be developed?

Co – chairs will write up the agenda w/ input of team and stakeholders.

  1. How will information/communications be disseminated to all SBM Team representatives?

Minutes/Notes taken and sent out in e-mail or hardcopy form.

  1. How will the proceedings of the meeting be disseminated to all stakeholders?

Decision Making Procedures
  1. Determine the system/process of how SBM Team decisions will be made.
  2. How will the SBM Team make decisions? What procedures will the team use?

Consensus Process is preferred to reach decisions. Consensus determined by when all stakeholders have an opportunity to share their thoughts and the will of the group is evident.

  1. How will the SBM Team involve all stakeholders in the decision making process?

Open forum for stakeholders will be part of every meeting.

Survey stakeholders as necessary on select issues.

  1. How will the SBM Team identify the effect the decision will have on all those involved?

Maintain an effective feedback loop.

Maintain balanced representation.

Report back to teams/groups for discussion and feedback.

Listen and keep lines of communication open.

Phone calls/Surveys to targeted or random parents for viewpoints and feedback as needed.

  1. How will decisions made by the SBM Team be shared with all of the stakeholders?

SBM Team Mission

All Site Based Management Teams must have a clearly defined mission. The mission of the team must: (a) be focused on student achievement, and (b) define the purpose of the SBM team.

What is the mission of the SBM Team?

Support Farnsworth staff and students to meet district proficiency levels.

An opportunity to bring all Farnsworth stakeholders together to create an environment of consensus that places students’ interests first.

Foster effective lines of communication with all stakeholders.

Decisions Team Will Make

Develop and implement action plans to meet district established proficiency levels using student proficiency data collected via district provided common assessment. (Use Student Achievement Action Plan Form)

Review current action plans and follow them.

What specific instructional, financial, or personnel decisions outlined in the SBM Decision Making Chart will the SBM Team make? For each decision selected, develop a specific action plan. (Use SBM Optional Action Plan Form)

Copies of the completed Planning Template need to be distributed to SBM Planning Team members prior to distribution to all staff.
