Acknowledgement of Risk and Release of Liability: I acknowledge that baseball/softball involve vigorous physical activity and will expose the player named above to certain risks including, but not limited to, resulting from surface hazards or equipment failures and otherwise associated with playing the game of baseball/softball. I, on behalf of the player named above and his/her heirs, executors, administrators and assigns, accept the above risks as well as any other risks incident to participating in SSLYBA baseball/softball. Furthermore, as parent or legal guardian of the player named above, for him/her and for his/her heirs, executors, administrators and assigns, I hereby release SSLYBA, its directors, trustees, officers, managers, coaches, agents, employees and volunteers from any liability whatsoever, for any injury, harm, loss, damage, or inconvenience, including but not limited to liability for consequential damages, which may be incurred as a result of player named above participating in baseball/softball games, practices, camps or events offered through SSLYBA.

Photo Release and Zero Tolerance Policy: I hereby grant full permission to SSLYBA to use the photograph, image or any other recording of the above-named player’s participation in SSLYBA baseball/softball games, practices, events and activities for any legitimate purpose. I, the undersigned parent/legal guardian of the player named above, agree that I and the player will abide by the rules of SSLYBA, its affiliated organizations and sponsors. I have read and understand “The Zero Tolerance Policy” of SSLYBA which is printed on the back of this form.

Consent for Medical Treatment/Disclosure of Medical Conditions: I have indicated below any medical condition below which I feel the coaches should be informed. I understand that I must be at practices or games if the player’s special condition requires it. As the parent/legal guardian of the above named player, I hereby give consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve the life, limb or well being of my dependent.

Special Notes:

Travel Ball is different from In-house (Rec Ball). You must inquire with a coach about getting on a team. There are no assurances you will make a team or get a certain amount of playing time.

~Travel age rule: Softball players and baseball players must be 11 years old to play travel ball. This is to support both the in-house and travel experience for the kids in the league and to ensure player preparedness.

~Any division may be cancelled due to availability or low registration.

~Teams/players are responsible for additional costs not covered by league – LEAGUE DOES NOT PAY EMERALD NECKLACE INSURANCE.

~Late sign ups are subject to a late fee and can be wait-listed for an opening.

~Coaches must complete a background check and concussion certification. See our website.

~All players are responsible for providing their own batting helmets. Softball divisions may require facemasks. Check with coach.

~From Peewee division up, baseball players should be wearing protective cups at all times.

~Practices generally begin in early April and games begin in May.

ZERO TOLERANCE POLICY:

1.  Any parent / fan entering the field of play for any reason will be required to leave the Volunteer Field Complex immediately.

2.  ANY FOUL OR OFFENSIVE LANGUAGE used by an individual and heard by an SSLYBA Board Member or Umpire will be cause for immediate removal from the Volunteer Field Complex. The individual will be suspended from attending functions at the Complex for a one-week period from the time of the offense.

3.  Any physical threats or intent to injure made by any player, coach, fan or board member in any way, shape or form will also be cause for immediate removal from the Volunteer Field Complex.

In summary, the Head Coach is ultimately responsible for the conduct of his or her players, coaches and fans. If you are asked to leave the Volunteer Field Complex for violations of policy and refuse to leave, your team will forfeit that game. The SSLYBA Board will reserve the right to review any violations and administer disciplinary actions up to and including a forfeit of game(s) and exclusion from participation in the Tournaments.

I recognize that the umpire is in complete charge of the game and for the sake of the players, his or her decision must be respected. If I have a complaint concerning specific umpires, I will register a complaint with the Head of Umpires. I will treat all coaches courteously and will not disagree with their coaching methods or decisions in front of the team members. Any parent-coach conferences will be held in private at the convenience of both parties. I will register all serious grievances concerning the program in writing to the SSLYBA Board. My behavior and language on the sidelines will set a good example for our children and will be of a positive and encouraging nature.

We depend on volunteers to keep the league running.
