The Senior Friendly HospitalACTION (Accelerating Change Together In Ontario) Program is an advanced leadership training program offered to all publicly-funded hospitals in Ontario for whom enhancing services to seniors is an important objective. As a result of participation in the SFH ACTION program, hospitals will have staff resources with knowledge, skills and expertise to optimize their senior friendly hospital achievements.For hospitals interested in participating, please complete the following registration form by September 20, 2015.
BACKGROUND
Frail older patients make up the majority of inpatient days in Ontario hospitals. The vision of the LHIN-led Ontario Senior Friendly Hospital (SFH) Strategy is to enable seniors to maintain optimal health and function while they are hospitalized so that they can transition successfully home or to the next appropriate level of care.
The pan-LHIN senior friendly hospital strategyis guided by the RGPs’ senior friendly hospital framework and the priorities identified in the 2011 provincial SFH report. Hospitals have made good progress in the priority areas - prevention and management of delirium and functional decline.To further build capacity and continue the momentum of the provincial strategy, we are offering adynamic training program informed by best practices in implementation science, quality improvement and change management. The program will feature internationally recognized speakers, facilitated group discussions and knowledge translation applied directly to senior friendly improvement plans. The SFH ACTION curriculum includes three themes:
1)Senior Friendly Hospital Framework: Embedding evidence-based approaches in the delivery of care to older adults requires a systemic approach that considers the influences of the entire care environment. The SFH Framework is an organization-wide approach that can be applied to planning and decision making, and adapted to the unique context of any hospital.
2)Quality Improvement Methodology:Quality Improvement is a systematic approach to making changes that lead to better patient outcomes, stronger system performance and enhanced professional development. Participants will develop an understanding on how to apply the Model of Improvement to SFH improvement strategies.
3)Change Management and Leadership: In a complex healthcare environment, the capacity to manage change is vital. Effective change management and leadership significantly influence the success rates of organizational initiatives. Participants will develop a range of strategies and tactics that will assist them in designing and implementing their SFH improvement goals.
LEARNING OBJECTIVES
At the end of the program, participants will be able to:
- Describe and use the five components of the SFH Framework as an organization-wide approach in service planning and care for seniors
- Understand and be able to apply the Model of Improvement to SFH improvement goal / aim
- Lead and engage others in embracing innovation and change
- Develop, implement, and sustain an improvement project, create an evaluation plan and monitor performance

WHO SHOULD ATTEND?
Publicly-funded hospitals in Ontario for whom enhancing senior friendly hospital care is an important strategic objective and who see the value of having an in-house resource team tolead its senior friendly hospital initiatives. Each participating hospital will be asked to nominate 2-4 staff members to participate in this leadership training opportunity and implement a SFH improvement goal. Participants could include a diverse representation of clinicians, support staff, or administrators. Four team members attending from each hospital would be considered ideal.
ELIGIBILITY AND PARTICIPATION CRITERIA
Hospitals participating in the SFH ACTION Programagree to:
- Allocate time and resources as needed to support team members’ attendance at learning events. This would include: transportation to in-person events, releasing and/or backfilling of staff time as required.
- Provide strategic and project support to ensure that SFH project goal is successful
- Ensure appropriate level of resources are available to the project (e.g. access to data, decision support, organizational development and quality improvement resources)
Teams participating in the SFH ACTION Programagree to:
- Actively participate in all learning events
- Submit program deliverables including project charter, progress updates, and end-of-project summary / case study
- Allocate their own time to support the implementation of a SFH project goal
- Complete evaluation feedback forms / interviews
COST
There is no cost to register for the program. All learning events and ongoing coaching will be offered complimentary to all participants. Hospitals are responsible for transportation to the in-person events. Accommodations will be covered for participants travelling from outside the Greater Toronto Area.

Accommodations are covered for participants travelling from outside the Greater Toronto Area at the conference hotel for the evenings prior to the 3-day workshop from Jan 20 to 22, 2016.

APPENDIX: SAMPLE SFH ACTION PROJECT GOALS
Teams may revise their project goals after the first learning event prior to completing the project charter. The ideal SFH project goals would be evidence-based, measurable, can be completed within a 10-month timeframe, and align with organizational priorities.
Examples:
- Increase rate of baseline CAM screening from 75% to 90% on unit A for patients age 65 and older
- Implement preventative strategies for all patients at risk of delirium on two hospital units
- Implement management strategies (order sets, care strategies) for patients screening positive for delirium on medical unit
- Implement CAM assessment on electronic chart to track rate of baseline delirium screening and incidence
- Develop/implement mechanism to engage patient/family in delirium prevention/management on medical unit
- Pilot early mobility initiative to reduce functional decline on one hospital unit
- Implement a night-time quiet program (e.g. reduce use of pagers and overhead announcements) to improve sleep of patientson two hospital units
- Conduct physical environment audit on 3 patient units to support delirium prevention, and implement at least 3 recommendations from the audit
- Implement a protocol to identify appropriate power of attorney/substitute decision maker for all patients on unit A
- Implement the Nutrition Risk Screening Tool on unit A in patients age 65 and older to prevent malnutrition
- Reduce unnecessary use of foley catheters by 10%
