Adoption Agreement
THIS AGREEMENT, made and entered into on by and between Save A Lab Rescue ("SAL") and Adopting Person(s) (“Adopter”).

Please initial indicating your acceptance of each term.
1.) Adopter agrees that the dog will be a house pet and will receive all of the care and attention necessary to ensure its health and wellbeing. Adopter shall provide adequate food, fresh water, indoor shelter, exercise, veterinary care and yearly inoculations as recommended by attending veterinarian, and shall abide by all state and local laws governing dogs. Adopter shall provide the dog with year around heartworm preventative and provide the dog with a yearly heartworm test. ADOPTER AGREES TO BRING HIS/HER ADOPTED LAB TO THEIR VETERINARIAN WITHIN 14 DAYS OF ADOPTION FOR A ROUTINE EXAM AND TO PURCHASE HEARTWORM PREVENTION, AND TO SUBMIT PROOF OF VISIT TO SAL.
2.) Adopter will not permit the dog to run free, off leash, or participate in any other behavior that results in the lab being out of the immediate control of the owner. When outside, the dog will NEVER be staked or chained out, or left unattended.
3.) Adopter agrees to provide adequate indoor sleeping quarters. Dogs are not to be left outside for unreasonable extended periods of time or overnight.
4.) Adopter will not transport the dog in an open pick up truck bed or in any other manner where the possibility of injury could occur.
5.) Adopter agrees that the dog will, at all time, wear a collar with current rabies tag and ID tag
6.) Adopter agrees that in the event of a hurricane evacuation, or any other government mandated evacuation, the Adopter will take the dog on the evacuation.
7.) Adopter agrees to contact SAL immediately if the dog becomes lost or is stolen.
8.) Adopter will notify SAL at least one week prior to any change in their contact information.
9.) Adopter will keep the dog for so long as it shall live and will not transfer ownership to another individual. The dog may not be transferred to any other person or organization for any reason.
10.) If custody of the dog must be relinquished, Adopter agrees to notify SAL immediately to arrange its transfer back to SAL. Adopter acknowledges and accepts that the adoption fee is NONREFUNDABLE.
11.) The Adopter agrees to take the dog as is, with all its defects, either observable or unobservable, and assumes full risk for the dog from the date of signing this agreement. Though care is given to assess physical condition and temperament of each dog that is accepted in SAL, and every possible effort is made to provide a dog which is compatible with the Adopter’s home, NO WARRANTY is made as to the physical condition or temperament of this dog.
HEARTWORM TREATMENT ADDENDUM
Note: This Addendum is only required if adopting a Heartworm Positive Dog.
Adopter understands that this dog has tested positive for heartworms and agrees to provide immiticide treatment as recommended by our chosen veterinarian. Adopter will be responsible for the cost of treatment and treatment will begin no later than 30 days from the date of adoption. Proof of treatment must be provided to SAL within seven (7) days from the start of treatment. In the event that the dog’s health does not allow this agreement to be honored, Save A Lab ("SAL") must be provided with a statement from Adopter’s veterinarian that this dog is not yet in physical or emotional condition for treatment. Based upon the veterinarian’s evaluation of this dog the Adopter will contact the veterinarian and establish the earliest date that the treatment can be administered.
It is the responsibility of the Adopter, not the veterinarian, to ensure that SAL has received verification that the treatment has been performed. Failure to comply with this agreement, unless otherwise agreed to in writing, will be considered a breach of the Contract.

SPAY/NEUTER ADDENDUM
Note: This Addendum is only required if adopting a dog who has not been spayed/neutered.
Proof of spay/neuter must be received from within fourteen (14) days after the surgery. In the event that the dog’s health does not allow this agreement to be honored, Save A Lab ("SAL") must be provided with a statement from Adopter’s veterinarian that this dog is not yet in physical or emotional condition for surgery. Based upon the veterinarian’s evaluation of this dog the Adopter will contact the veterinarian and establish the earliest date that the procedure can be performed.
It is the responsibility of the Adopter, not the veterinarian, to ensure that SAL has received verification that the surgery has been performed. Failure to comply with this agreement, unless otherwise agreed to in writing, will be considered a breach of the Contract.
Adopter agrees to have this dog spayed/neutered at adopter’s expense by the time the dog is 6 months old, and understands that this spay/neuter agreement is an agreement that this dog will not produce a litter of puppies either as purposely bred or by accidental breeding. Adopter also agrees that should the puppy/dog not be altered, Adopter will return said puppy/dog back to SAL with no refund.
Adopter Signature:
SAL reserves the right to immediately remove and reclaim the dog from the Adopter(s) if any of the above conditions are violated in any way or it is found that the dog is being abused, neglected, or mistreated. If SAL removes and reclaims the dog, Adopter shall pay SAL all costs associated with the removal and reclamation, including but not limited to, veterinary bills, attorney bills, and any court costs.
Adopter(s) understands that this adoption is NOT final until this agreement has been completed in its entirety and the adoption fee has been paid to SAL.
As of the date of this agreement, the Adopter(s) assumes responsibility for the dog’s actions and shall hold SAL and its representatives FREE from all claims for personal injury to others, and/or property damage associated with the adoption.
