FYS Programs / 2012-2013 Update / School Sites
Our Community, Our Children Recruitment Campaign / CPC/SFUSD Phone line supports the recruitment of NREFM homes at school sites.
Two staff will provide child specific permanency work and school/community outreach to support the recruitment of foster/adoptive families, respite providers, and mentors. / At every SFUSD school
Foster Youth Mentoring for Success / School-based mentoring program. Mentors are school based staff or community volunteers. Mentees meet at least weekly with mentors and participate in monthly group activities. / Elementary: Carmichael, Bret Harte, Cobb, El Dorado, John Muir, Malcolm X, New Traditions, Paul Revere, Rooftop, Rosa Parks, Sheridan, Fairmont, Carver, Tenderloin, Visitacion Valley Elementary
Middle: Aptos, Denman, Everett, Francisco, Giannini, Hoover, Horace Mann, ISA, Lick,Marina, MLK, Paul Revere, Presidio, Visitacion Valley
High: Balboa, Burton, Civic Center, Hilltop, ISA, Marshall, Mission, O’Connell, SOTA, Wallenberg
Foster Youth Services Liaisons / FYS Liaisons (FYSL) are responsible for facilitating communication between school staff and the youth’s care providers. They will also monitor academic status, provide/prioritize referrals, and provide other related services. / At every SFUSD school
Tutoring / SFUSD FYS subcontracts with tutoring providers to provide weekly individualized tutoring to foster youth students. Priority is given to youth performing below grade level and youth unable to access other tutoring programs. / At every SFUSD school
Educational Case Management / Graduate level MSW interns provide individual and group interventions with foster youth students, attend school and other collaborative meetings, and consult with multidisciplinary team members. / Elementary Schools: To be determined
Middle Schools: To be determined
High Schools: Downtown, ISA, Ida B Wells, Lincoln, Mission, O’Connell, Thurgood Marshall, Washington
Enrollment/ Transfer of Records / FYS can assist with immediate enrollment and timely transfer of school records for current and prior SFUSD students. Requests should be made with the 1144/1145 forms.
Juvenile Detention Program / FYS JD Liaisons support the academic achievement of foster youth students residing in juvenile detention facilities or transitioning to/from a facility. / Log Cabin Ranch, Woodside Learning Center
Guardian Scholars Summer Academy / Students attend City College and receive high school and college credits for participating in courses related to college success and career exploration. The program provides free tuition, meals, books, supplies, and transportation passes. / Available to 11th, 12th or entering college freshman
Applications will be available in February 2011
Professional Development / FYS staff and community partners are available to provide training for social workers, care providers, school site staff, service providers and others on the educational needs of foster youth and available resources. / Contact FYS directly to schedule training at your site.
Team Decision Making and GOALS Meetings / School staff attend to provide updates on how the student is doing in school and to strategize school interventions to support the current home placement or provide stability during a transition to a new home and/or school / At every SFUSD school
