Effective today, January 31, 2004, you are terminated from your position of Computer Consultant/Advisor in the Department of Forestry and Environmental Studies, CornellUniversity. This Decision is based on the numerous discussions that we have had during the past eighteen months regarding the unacceptable level of your performance-specifically, your failure to work independently,to meet project deadlines and to maintain effective work relationships with your colleagues and clients. You inability to meet expectations in these areas have resulted in three letters of warning, an overall unsatisfactory performance dialogue rating last year, and as a result, no salary increase this year.

(ReturnUniversity Property)

You are expected to immediately return all property belonging to the department and the university, including procurement card, keys, equipment, staff ID card, parking permit, laptop computer, etc.

(Vacation Accrual, Final Pay Check, Benefits)

Your accrued vacation will be paid to you in your final check. You will receive information from Records Administration concerning your eligibility to continue health insurance. Any other questions regarding the status of your benefits should be directed to the Benefit Services office at 255-3936.
