2 / Shift Supervisor / Call Community Guide from Guide Call List to assist with intake
3 / Guide / Guide reports to police station in a timely manner. Guides are responsible for their own transportation to and from police station.
4 / Shift Supervisor / Advise dispatch that a community guide has been called out and will be arriving on station.
5 / Guide / Upon arrival, report to Clerk’s Office, located at the front window of station, just inside entrance. Clerk’s Office business hoursare M-F 7a-6p. During other times use red phone in lobby to advise Southwest Dispatch in Strongsville of your arrival

Decision: Once Guide arrives at station, report to intake officer for further instructions

Benchmarks:

  • Guide available to assist
  • Guide provides own transportation
  • Guide respond in a timely manner

Intake: Stage where intake information packet is completed.

Welcome the participant to the program, treat them with dignity and respect and bring them to the interview room to complete intake packet
7 / Intake Officer / Ascertain if participant possesses any contraband they wish to relinquish while utilizing prudent officer safety measures
8 / Intake Officer / While Guide is enroute to the station, begin intake packet
  • Program Participant Intake Form
  • Participation Agreement
  • DAST (Drug Abuse Screening Test)
  • Make Copies of Identification and Insurance card
  • If no identification, officer to verify identity
  • Of special note is to inquire about probation status

9 / Intake Officer / Verify participant eligibility through LEADS and CCH
10 / Intake Officer / Intake Officer and Guide will discuss paperwork completed and any pertinent information regarding participant
11 / Guide / Build rapport with participant while completing forms (welcome them to program; treat them with respect and dignity)

Decision: If the participant is eligible, move on to form completion. If the participant is ineligible, provide list of treatment centers before they leave and place completed intake paperwork in Sgt. Greenhill’s mailbox.

Benchmarks:

  • Guide develops a positive working relationship with Officers
  • Guide is able to review documentation and complete any missing sections
  • Guide is able to show compassion, patience and caring while being professional

Form completion: Stage where all forms are reviewed, copies have been completed and information gathered necessary to place participant in appropriate detox/treatment facility.

Review and verify intake packet has been completed
13 / Guide / Pay special attention to ensure copy is made of participant ID and insurance card (if available)
14 / Guide / During packet completion, key areas for attention are:
  • Need for detox and/or treatment
  • Stability of support system
  • Availability of transportation for participant to detox and/or treatment

15 / Guide / Verify that participant is ready to accept and commit to a treatment program, including following all rules of treatment facilities and if they are willing to travel to an out of state facility
  • Make note in the comment section of the Program Participant Intake Form regarding willingness to travel out of state
  • If they are willing to travel out of state, make note of ability to cover travel expenses

Decision: If participant is eligible, the information packet has been completed, they are committed to a detox and treatment program, move on to next stage Detox/Treatment Placement. If they are not willing to commit to following treatment center rules or have otherwise been deemed ineligible, provide them with list of treatment centers before they leave.

Benchmarks:

  • Guide ensures all paperwork completed
  • All copies of ID and insurance are included (if available)
  • Guide able to build rapport with participants and elicit necessary information

Detox/Treatment Placement: Stage where participant is placed or referred to detox/treatment facility.

Use Detox/Treatment Center List to determine appropriate placement for participant
  • Most detox/treatment center intake hours are M-F from 8a-4p

17 / Guide / Once a detox/treatment center has been selected, call facility to attempt placement (if intake is done during business hours)
  • Detox placement comes first (if necessary)

18 / Guide / Note on Program Participant Intake Form whether participant was admitted for detox/treatment or not

Decision: If participant was placed for detox/treatment immediately, move on to transportation section. If participant was not admitted immediately, move to conclusion.

Benchmarks:

  • Guide uses Detox/Treatment Center list appropriately
  • Guide completes phone calls for placement with selected facility (where applicable)
  • Guide notes appropriate information on Program Participation Intake Form for Treatment Liaison Coordinator to make follow-up calls for placement (if necessary)

Transportation: Stage where participant is either transported to appropriate facility or allowed to leave station to await placement for treatment.

Determine availability of transportation

20 / Guide / Arrange for transportation to treatment facility through the above sources
  • Policy dictates that if Guide is providing transportation, a minimum of two people, including one offier
  • Guide should consult with Shift Supervisor regarding transportation if carried out by Guide

21 / Guide / If transporting participant, go directly to treatment center with no stops along the route
22 / Guide / If participant provides own transportation, the intake is complete and completed intake packet is placed in Sgt. Greenhill’s mailbox
  • Note transportation method on the Program Participant Intake Form

Decision: If participant provides own transportation, or Guide provides transportation, the intake is complete and all intake forms are to be returned to Sgt. Greenhill’s mailbox.

Benchmarks:

  • Guide follows procedure of using 2 people for transports
  • Guide follows procedure of no stops along the route
  • Guide ensures participant has some form of transportation to treatment

Conclusion: Stage where either transportation has been completed or participant has been sent home to await placement.

If participant is not immediately transported to treatment facility, place all completed paperwork in Sgt. Greenhill’s mailbox
Please note:
  • Any calls to treatment centers made
  • Any pertinent issues that may affect placement, such as unwillingness to leave Ohio for treatment

Decision: Has all intake paperwork been completed and all available avenues for treatment exhausted, if so, intake is complete and forms to be returned to Sgt. Greenhill’s mailbox. If there are options still available, refer back to previous stages for guidance.

Benchmarks:

  • Guide has exhausted all available treatment options
  • Guide has reviewed all paperwork for completeness
  • Guide has placed all completed forms in appropriate location
