Business Rules-Written:
1. Applies only to active employees or inactive employees who terminated after the "go live" date of the agency where the employee was last employed.
2. Employee must submit written authorization including his/her signature and employee ID number or SSN. This information may be included on the requestor's letterhead or on a separate form.
3. Employment verifications may be faxed to the People First Service Center for processing under the following conditions:
  • The verification request is faxed through the agency Human Resources office by an H role.
  • The verification request is accompanied by a release form provided by the People First Service Center with the employee's printed name, signature, and People First Identification number.
  • The verification request is accompanied by a release form provided by the People First Service Center with the H role's printed name, signature, and People First Identification number.
  • The verification request can be faxed for processing to fax number (850) 219-3446.

Guidelines:
The employee will generate a PIN number in the IVR system. To complete this process, the employee does the following:

After the Employee has obtained a PIN, the employee should give it to the vendor. The vendor should do the following:

Employee’s work information is divided into two categories, key employment dates and salary information. These are outlined below.
  1. Key Employment Dates - Includes State Hire date / Continuous Service Date, Separation Date and Inactive Date. This information will include the Employment status, i.e, if the employee still works with the State.
  2. Salary Information - Includes Base Salary only (does not include OT, bonuses, etc.), Date of Hire information, Employment Status and Pay frequency.
