2. DESCRIPTION OF PROJECT PROPOSAL

Attach a typed project description (maximum 2-3 pages) describing the proposed project under all of the following seven numbered headings:

  1. Proposed Project – summary of goals and deliverables
  2. Implementation Plan – basic outline of how project will be implemented
  3. Ownership – who will be the final owner of any project equipment/materials/supplies? (Cannot be a Rotary Club or Rotarian)
  4. Expenditure Budget – summary of projectedexpenditures budget(include some detail regarding expenses)
  5. Revenue Budget– summary of projected revenue budget (list funding sources and proposed monetarycontributions, including amount ofrequested District Grant funds AND 50% Club match)
  6. Start Date - estimated start date of project
  7. Completion Date - estimated completion date of project(project MUST be completed within 12 months)

4. ROTARY CLUB PROJECT COMMITTEE

This committee is responsible to coordinate the project, monitor funds and provide reporting and financial accounting to the District 7530 Grants Committee. 5. TERMSAND CONDITIONS FOR THE DISTRICT 7530 DISTRICT GRANT

NOTE: Only “qualified” Clubs are eligible to participate in District Grant programs. To be qualified, at least one Club member (preferably the Club President and/or Club Foundation Chair) must attend a Grants Management Training Seminar and the Club’s President and President-Elect must sign and submit the required Club M.O.U.

(Memorandum of Understanding).

6. ROTARY CLUB COMMITTMENT

7. APPLICATION FORM COMPLETION CHECKLIST

Please complete all sections of the form and note that incomplete applications will not be considered. District Grant funds are normally limited to approximately$1,000 but may, at District discretion, be funded for an additional amount.

*** Every effort will be made to award available funds in the fairest manner possible with all applications reviewed by the District Grants Committee.

*** Do not start the project until you receive written funding approval of your grant application.

Before submitting this application, please review this checklist and make sure that your application is complete.
