PLEASE READ THE FOLLOWING REQUIREMENTS FOR USE OF THE CLUBHOUSE, INITIAL WHERE APPROPRIATE AND SIGN AT THE BOTTOM OF PAGE 2:

To reserve the Clubhouse, contact the Association Manager as early as possible to ensureyour requested date is available. Please reserve holidays at least two months in advance. Two separate checks, one for $60.00 (Reservation Fee) and one for $100.00 (Security Deposit) made payable to “Rock Lake Finance”, must be remitted at least two weeks prior to your function. Read, initial and signthis Reservation Agreement then seal it along with the two checks (mentioned above) in an envelope. Deposit the envelope in the “Dues Box” located immediately inside the Clubhouse entrance, again, at least two weeks in advance. Proper cleaning of the Clubhouse immediately following your event will ensure the prompt return of your Security Deposit within ten (10) days. Please remove the Clubhouse Cleaning Checklist attachedand use it at the end of your event.

Residents only may reserve the Clubhouse facilitiesfor private social functions. No commercial or “for profit” functions permitted. Any renter intending to provide/server alcohol of any kind during anevent will assume full responsibility for providing and serving adult beverages. The Rock Lake North Association will never provide alcohol and therefore will not accept liability for individuals providing and/or consuming alcoholof any kind,nor for their resulting actions, while on the premises. ______(initial here)

If you are reserving the Clubhouse on behalf of a family member(or members)who do notpermanently reside at Rock Lake North, you (or a resident on your behalffrom the Rock Lake community)must be present at all timesduring thefunction or event. ______(initial here)

Children are not permitted downstairs (lower level) during your event. No childrenpermitted to use/ride the chair lift. If children are caught playing on the lower level of the Clubhouse or the chair lift, your entire security deposit will be forfeited. Use of the playground equipment is at your own risk! Parents are required to supervise children visiting the facility at all times. ______(initial here)

The maximum occupancy of the Clubhouse cannot exceed 100 people - no exceptions! The Clubhouse must be cleaned and vacated by (no later than) 1:00am. ______(initial here)

Clubhouse -Our Clubhouse is a smoke-free facility. Smokingpermitted outside or on the side patio as long as it is not offending residents or other guests. All functions will be of a social nature and conducted as if it were taking place in your own home. Guests will have use of the meeting hall, kitchen, restrooms, patio and outside recreation area (weather permitting). No onepermitted on the lower level (downstairs) during your event. The resident renting theClubhouse is responsible for ensuring these rules are strictly enforced. ______(initial here)

Patio Grill -The patio grill has a padlock. If you desire use of the grill for your event, you must notify the Association Manager(or a member of the Board)in advance to obtain a key to the lock. Prompt return of the key will ensure prompt return of your security deposit. You must thoroughly clean the grill after its use or you will forfeit $25.00of your security deposit as a cleaning fee. Rentersreceiving the key will have a $10 fee deducted from their security deposit to cover the cost of propane used. Only if you intend to use thegrill during your event, initial here______.

Kitchen-The equipment and supplies in the kitchen belong to the Homeowners’ Association;however, are available for use by the renter(s). All dishes, glasses, cookware, utensils, etc, that were used must be cleaned and returned to their designated cupboards and drawers. Thoroughly clean the stovetop/oven, small appliances, countersand all other surfaces. The renter must furnish any paper products, plastic glasses or cups, coffee, condiments, hand towels, soap, etc.

Tables and Chairs –Renters may arrange tables and chairsto meet their needs. However, please return tables and chairs to their original locations and wiped clean if necessary. Place chairs upside down on top of the tables prior to ensure thorough cleaning/sweeping ofthe floor.

Audio/Video Equipment - Renters may use of the audio and video systemsduring their functions. As further explained in the “Damageto Property” section, the renter will assume full responsibility for the equipment, and will be liable for the costs to repair or replace any components that sustain damagedduring their scheduled use of the clubhouse. Only if you intend to use the audio/video equipment during your event, initial here______. The Association Managerwill provideaccess to the audio cabinet. If the Manager is unavailable, please contact a member of the Board.

Floors -Pleasedo not drag tables or chairs across the floor - doing so couldseverely damage the surface. Floors must be spot cleaned and swept/vacuumed thoroughly after your event.

Damage to Property -The renter is responsible for any (and all) damage to equipment, furnishingsor the Clubhouse building itself during an event. If any damage occurs, the renter must remit payment to repair the damage, or replace thedamaged item(s) with one(s) of the same quality within thirty(30) days of the date of this agreement. Failure to take financial responsibility for any damages to the Clubhouse and/or its contents will result in legal action and refusal of any future rental requests. ______(initial here)

Emergencies-Nopublic telephone service or emergency call button is available insideof the Clubhouse. In the event of an emergency, have a cell phone user DIAL 9-1-1 IMMEDIATELY! TheClubhouse address is 113 Rock Lake Drive. The HOA will not be responsible for any bills/fees resulting from emergency services responding for emergencies unrelated to the facility.

Other –No wall-hanging decorationspermitted. No use of glitter and/or confetti permitted. Please remove all decorations immediately following your function. Place all empty cans and/or bottles in the recycle container(s) and take it/them outside. Please seal all trash bags and place them inside the trash containers on the patio. The Clubhouse doors lock promptly at 10:00 PM; however, renting guests may re-enter. Please remember, if you serve alcohol during your function, you assume full liability! You also assume full liability for any guest injured by unauthorized use ofthe exercise equipment. Once again, the lower level is reserved for resident use only! ______(initial here)

Clubhouse Cleaning Checklist

(Please detach and keep this checklistbefore returning your signed Reservation Agreement to use as a reference after your event)

Prior to your event, if you find that an area is unclean, or there is an issue or problem with any of the equipment or the facility itself,please contact the Association Managerimmediately so that you will not be held responsible! We will thoroughly inspect the Clubhousefollowing each scheduled event; to ensure the prompt return of your security deposit, thoroughly clean the Clubhouse immediately following your eventand restore itto the same condition it was in just prior to your use. Remove and take this checklist with you on the day of your event to use as a reference for your convenience.

Place all paper trash, unwanted food, etc. in the large trashcontainer by the kitchen
entrance. After your event, please empty that container into the large trash
containers located on the far right of the outside patio and install a new trash bag.
(Please do not place recyclables in the regulartrash containers)

Place all empty soda cans, beer cans, plastic bottles, unwanted plastic food containers,
etc.in the recycle containers located on either side of the exit door to the patio. After your event, please empty those containers into the large recycle bin on
the far right of the patio near the large trash containers.
(Please do not place regular trash in the recycle bins)

Vacuum carpets thoroughly. Sweep hard-surface floors thoroughly. Please spot
clean any food stains and/or spills if necessary.

You may arrange tables and chairs to meet the need(s) for your event. However,
please return all tables and chairs to their original locationsand wipe them
clean before you leave. Please carry tables and chairs when moving them!
You will be responsible for any damage to the carpeting or parquet floor resulting
from dragging tables/chairs across the floor!

Clean the kitchen thoroughly. This includes floors, countertops, stovetop/oven and
any appliancesused. Cookware, dishes, utensils, etc. shall be hand-washed or
placed in thedishwasher and the wash cycle started before you leave. The bar
area must alsobe wiped clean and the ice bin emptied (if used).

Clean the bathroomsand empty their trashcontainers into the large trash
containers (on the far right of the patio) at the end of your event. Return any toiletries
moved/removed from the bathrooms to their original location at the end of your event.

If you used the grill on the patio, please thoroughly clean all cooking surfaces after
use oryouwill forfeit $25 of your security deposit as a cleaning fee.

Following a few simple steps will ensure the return of your entire security deposit and will keep our Clubhouse clean for the next person or group who needs to use it. We greatly appreciate your help! Thank you - and enjoy your event!
