Welcome to Camp Shalom 2018! We know there is a lot of information to look at regarding Camp Shalom, but please take the time to download the forms and complete the necessary forms. These forms can also be viewed on your CampSite account as well. Please note, this is for upper Camp Shalom (kindergarten and older) Yeladim forms have been emailed to you.

It is a state requirement that all forms must be completed prior to the start of camp. Don’t forget we also need the Florida Certification of Immunization and Student Examination Form from your child’s pediatrician. Please don’t hesitate to contact the Camp Office (561-712-5227) if you have any questions, or would prefer a mailed copy of these forms.

Circus Camp Letter- This provides information regarding Circus Camp. For those participating in Circus, it is necessary to read this letter.

Camp Shalom Outfitters – In case you want to get some cool Camp Shalom merchandise for your camper. It is completely optional to purchase items from the Camp Shalom Store

Group Request Form- Use this form to communicate the group requests for your camper.

Swim Information- Please read this form if you have a camper entering 2nd grade and above…we think it’s good news about swimming

Medication Authorization: If your camper takes medicine during the camp day this form must be completed. If your camper takes prescription medication during the day it must be signed by your child’s physician.

Nut Free Zone- Please read this flyer to find out about the new Camp Shalom Nut policy.

Carpool Procedures- Includes procedures for drop off and pick up at camp.

Kindergarten Waivers- All campers entering kindergarten who are attending camp week 2 will be required to complete the Circus waiver. **includes both Nitzan and Sports K**

K’Tanim Waivers- These are the waivers for campers in the K’Tanim unit.

Kesher Waivers-These are the waivers for campers in the Kesher unit.

Kochavim Waivers-These are the waivers for the campers in the Kochavim Unit

Tikvah/Shining Stars Waivers-These are the waivers for campers in Tikvah, Shining Stars 1 and Shining Stars 2.

Sports 1 Waivers- These are the waivers for campers in the Sports 1 group.

Sports 23 Waivers- These are the waivers for campers in the Sports 23 group.
