In submitting this application, I understand that I must meet the following criteria:
1. My current six (6) semester grade point average must be 2.75 or higher on a 4.0 scale (or its equivalent).
2. I must submit an official transcript, which must be signed by the high school guidance counselor, showing this grade point average and all high school grades with this application.
3. I must be a 2015 graduating senior of one of the following high schools: Batesville High School (and a resident of Ripley County and/or the City of Batesville); East Central High School (and a resident of Ripley County); Jac-Cen-Del High School; Milan High School; Rising Sun High School; South Dearborn High School (and a resident of the City of Aurora); South Ripley High School; or a graduating senior who attends a private and/or parochial school and currently a resident of either Ohio County, Ripley County, or the City of Aurora.
4. I will execute a contract to properly use said award, pursuant to the standards and procedures set forth by the Rising Sun Regional Foundation, Inc. (RSRF), which includes the use of said award at any institution that is accredited for higher education.
5. I will submit this completed application and an official transcript of grades to the RSRF office on or before the close of business (4:00 p.m. EST) Wednesday, April 15, 2015.

Scholarship Contract
For and in consideration of my selection as the recipient of the Rising Sun Regional Foundation, Inc. Scholarship Award for 2015 and payment of said award for my benefit, I hereby agree as follows, to-wit:
1. I agree to furnish proof of my acceptance and enrollment in an institution accredited for higher education.
2. I agree to utilize this grant at the institution so stated by me on the application form, or will consult RSRF regarding any proposed change.
3. I understand that I must use said funds within one calendar year from the date of the award or any unused balance will be forfeited and I understand that the award may be used for tuition, books, room and board, or student fees but that I will first apply said funds toward any outstanding tuition and book expenses.
4. I hereby specifically waive any confidentiality which might otherwise be involved and freely consent to said institution furnishing any and all scholastic information regarding me which it may at any time have in its file or on record.
