IV. Sample Deliverables to be provided by a Regional Demonstration Center for NW Florida:
The following is a sample of itemized deliverables the applicant is required to provide the BW Florida Region.
Use the Rehabilitation Services Administration (RSA) approved data collection instrument and RSA procedures to collect and establish baseline data.
PUBLIC AWARENESS ACTIVITIES
• Applicant shall conduct 40 AT skills-development trainings in assessing the need for assistive technology devices and services.
• Applicant shall conduct a minimum of 19 structured presentations at educational conferences, exhibits and special disability specific events for demonstrating the use and applicability of assistive technology devices and services.
• Applicant shall at a minimum participate in 12 educational conferences and exhibit fairs.
• Applicant shall conduct a minimum of 4 IT and Telecommunications trainings. Trainings include telecommunication devices, a common carrier or carrier of telecommunications, Telecommunications Devicesfor Individuals with Deafness (TDD), and telecommunications relay services.
• Applicant shall provide Information and Referral services to a minimum of 120 consumers, families and other interested parties such as state and local governments, grassroots disability organizations, not-for-profitagencies serving individuals with disabilities and elders with disabilities, workforce investment systems, Statevocational rehabilitation centers, public and private employers, elementary, secondary and post-secondary public schools; and social, medical, educational, employment, and transportation services for individuals with disabilities.
• Applicant shall conduct Device Specific trainings to a minimum of 998 individuals.
• Applicant shall conduct at minimum 4 Transition trainings and/or outreach activities to students, parents and school personnel related to assistive technology.
• Applicant shall provide 1 website update per month for promotion of assistive technology in their region. This totals 12 for the year.
ACCESS TO ASSISTIVE TECHNOLOGY
• Applicant shall purchase a minimum of 20 new devices for the purpose of loaning to consumers and professionals within their county catchment area.
• Applicant shall provide assistive technology equipment loans to a minimum of 100 consumers.
• Applicant shall conduct a minimum of 890 direct device demonstrations on the use of assistive technology devices and services for consumers.
• Applicant shall conduct a minimum of 4 indirect device demonstrations on the use and applicability of assistive technology devices and services to consumers.
• Improving Access to AT in Education to a minimum of 114 consumers.
• Improving Access to AT in Employment to a minimum of 27 consumers.
• Improving Access to AT in Community Living to a minimum of 95 consumers.
CONTRACT MANAGEMENT & TECHNICAL ASSISTANCE
• Applicant shall participate in the FAAST Contract Summit.
• Applicant shall at a minimum participate in 2 FAAST Board of Director/Assistive Technology AdvisoryCouncil, ATAC meetings.
• Applicant shall participate in monthly Regional Demonstration Center teleconferences.
• Applicant hall participate in 1 Face to Face Meeting.
• Applicant all participate in the FAAST Legislative Workday held in Tallahassee, FL.

Bidders are required to submit three (3) references for whom similar services have been performed as those requested in this RFP. FAAST, Inc. will contact two of the three references provided to obtain an assessment of the applicant’s past performance. For applicants with a history of contracting with FAAST, Inc. one of the references may be FAAST, Inc. assessment of performance completed at the conclusion of the previous contract. Three attempts will be made to contact each reference.
