Surgical Rotation: Burns/Plastic Surgery (Harborview Medical Center Burn Unit)

Length of Rotation: Four Weeks

Goals and Objectives

Patient care:

·  Demonstrate the ability to manage and care for burns following the guidelines in Burn Pearls, the rotation handbook

·  Demonstrate the ability to assess and manage the burn patient including making a plan and assuring its implantation in consultation with upper level members of the team

·  Demonstrate the ability to assess and manage a simple lacerations and the ability to evaluate complex lacerations and wounds

Technical skill:

·  Demonstrate technical proficiency in the emergency room and operating room with basic wound suturing, burn care, debridement, skin grafting and appropriate dressings

·  Master venous access, airway management and fluid resuscitation of the burn patient

·  Demonstrate the principles of burn wound management including escharotomy, skin grafting, coverage techniques and debridement techniques including revision of scars and resultant deformities and composite tissue transfers.

Medical knowledge:

·  Demonstrate mastering the pathophysiology of the burn injury, including the dynamics of thermal injury and progression of tissue damage, including how to care for: hypothermia, frostbite, flame burns, scald burns, electrical burns, chemical burns, smoke inhalation injury and carbon monoxide poisoning

·  Learn to examination and assess a burn wound including extent and depth of injury

·  Learn the principles of burn shock and fluid resuscitation

·  Learn bacteriology and immunologic alteration of the burn wound

·  Learn the indications and techniques of burn wound immobilization and coverage

·  Learn about the rehabilitation of the burn wound patient

·  Demonstrate the ability to perform a basic craniofacial trauma evaluation, including important elements of history and physical examination

·  Demonstrate the ability to identify fractures on the craniofacial CT scan

·  Demonstrate the ability to perform a basic hand examination and evaluate common hand injuries in the emergency department and clinic

·  Demonstrate understanding of basic hand anatomy

Practice-based learning and improvement:

·  Demonstrate understanding of the expected course of wound healing by regular attendance at wound rounds and observation and participation in the wound care of once patient's

Interpersonal and communication skills:

·  All residents should again an appreciation for a multidisciplinary and holistic approach to the care of the burn patient

Professionalism:

·  All residents should gain an appreciation for a multidisciplinary and holistic approach to patient care

·  Demonstrate sensitivity to age, gender and culture of patients and other health care professionals

Systems-based practice:

·  Demonstrate an appreciation for the team approach a patient care and presenting the patients at multidisciplinary rounds on Mondays and Thursdays
