Use these Reporting Forms to report on the Expected Results, the Approved Budget and any Grant Conditions that may have been attached to your grant.

Please contact your Program Manager or Grants Associate before you reallocate funds in your approved budget.

Using point form, clearly explain your use of your grant funds and the progress made toward achieving your grant’s expected results as measured by the performance indicators outlined in Schedule A of your Letter of Agreement. (There will be an opportunity to describe your project in more detail in question C2.)

In Column 1, copy the Expected Results from Schedule A of your Letter of Agreement. This is what you are hoping to achieve through your grant.

In Column 2, copy the Performance Indicators from Schedule A of your Letter of Agreement. This is what you are measuring or tracking so that you will know that you are achieving your expected results. Note: For grants awarded before 2006, please skip this column.

Conduct an analysis of the pilot and a plan to ensure the most efficient and effective way to sustain the service
Expand the network of transportation supporters by 46% / Agreement signed between EKIOC and Lanark Transportation on Februrary 1/10
Provided 1755 rides during the grant period which is 88% of expected results.
An analysis was completed, please see Appendix A.
Initial transportation supporters included 48 organizations. Upon completion of project an additional 31 organizations/businesses support transportation. Therefore we expand the network by 65%.
Improved the quality of life for rural citizens of North and South Leeds / 95% of a randon sampling of participants will indicate improved quality of life through evaluation surveys / A random sampling of participants were asked "Has this transportation service had a positive impact on your life? How so?"
91% responded that this service impacted their lives positively. Below are examples of some answers as to how it affect them.
1.The service has changed XX life he is happier. He is able to do more things. He is happy to get up in the morning and go to work. He really liked his driver. The service was very accommodating with last minute changes.
2. It made a big difference, I needed transport at this time; it would have been difficult to manage at the time without it.
3. It was definitely a positive experience, I have Doctor and business appointments in Ottawa. I no longer have a driver’s license because I cannot see making it difficult to travel even if I took a bus or the train I would need someone to travel with me. My daughter has her own business and is very busy making it difficult for her to provide the ride. Without the service I would have difficulty to and from Ottawa.

I would have had difficulty attending school.
