REGISTRATION FOR THE RIDE

Project 22 Ride, Film and Resource Event

We are excited to have you join us on Friday, September 16, 2016 for the Project 22 Ride, Film and Resource Event.

Complete the Registration and Release/Consent forms. Please take time to read the Rules of the Road, and Route Map with turn by turn directions over carefully. This information about your motorcycle, special needs and possible passenger is required to keep all riders safe. It is important that these forms are filled out completely and that they are signed by the driver of the motorcycle.

Following the motorcycle ride, we welcome you to join us at the Milwaukee Harley-Davidson Museum, for a BBQ buffet, resource exposition, screening of the film Project 22, followed by a panel discussion.

We look forward to sharing the ride for awareness, the eye-opening film and wonderful resources.

PROJECT 22 MOTORCYCLE ACCIDENT WAIVER AND RELEASE OF ALL LIABILITY

I acknowledge that motorcycle activity is a potentially hazardous activity which can be a test of a person’s physical and mental limits and carries with a potential for death, serious injury, and property loss. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of equipment, vehicular traffic, and actions of other people. These risks are not only inherent to riders, but also present for passengers, spectators, and volunteers. I hereby assume all of the risks of participating in this event.

I realize that liability may arise from negligence or carelessness on the part of the persons or entities organizing or conducting this event and hereby release them of all possible liability. In consideration of the acceptance of the right to participate, entrants and passengers, by execution of this entry form, release and discharge the University of Wisconsin – Milwaukee Military and Veteran Resource Center and its affiliates and all other sponsors and their officers, directors, trustees, employees, agents, representatives, members, volunteers, and anyone else connected with the management of Project 22 A Ride, Film and Resource Event from any and all known or unknown damages, injuries, losses and claims from any cause whatsoever that may be suffered by the entrant and passenger to his/her person or property.

Project 22: A Ride, Film & Resource Event

Rules of the Road

1.  Check bike before the ride to make sure it is mechanically sound (lights, signals, horn, etc.)

2.  No illegal drugs or alcohol while riding in formation.

3.  Staggered formation 2-3 seconds following distance. Maintain a steady pace and distance between bikes or vehicles.

4.  If you fall out of formation for any reason (Example: for an early fuel stop) do not try to catch up, make your way to the Harley-Davidson Museum based on speed limits (400 W Canal St, Milwaukee, WI 53201).

5.  Use basic hand signals in compliance with legal rules of the road.

6.  When on divided highway always use right lane, unless directed by law enforcement to do otherwise.

7.  Obey all speed and traffic laws.

8.  NO HOT DOGGING will be tolerated, for example: burn outs, weaving in and out, wheelies.

9.  Safety Captains for each group, will be wearing green safety vests. Anyone that has a bike failure or any accidents will be handled by them. Do not panic-stop in case another bike drops out.

10.  Road Captains for each group, will be marked with orange arm bands. One will lead and one will follow at the end of the group.

11.  Follow all directions of all of the Captains along the route.

12.  Be aware of changing road conditions especially at intersections. Be prepared for changing weather conditions and drive safely.

13.  All riders will follow the traffic signals of traffic officers located at intersections along the route.

Remember this ride is all about suicide prevention and awareness and

in memory of those who have lost the battle at home.
