Regional Hospital Contract Deliverables

Note: KHERF will use their own internal process for quarterly work plan status reports and tracking deliverable completion. Reimbursement Request Forms will be submitted to KHERF on a quarterly basis. The regional hospital will ensure all contract funds are expended by June 30, 2015.
Description of Tasks / Progress
1 / Coordinate and conduct regional healthcare coalition meetings at least quarterly. Hospital Regional Coordinators should continue to encourage the involvement of general health and medical partners through invitations to participate in coalition activities. Quarterly coalition meeting sign-in sheets and meeting minutes/summaries will be provided to KHERF within 10 working days of each regional healthcare coalition meeting. 2 / Facilitate discussion and provide answers to the HPP Program Measures, the Healthcare Coalition Developmental Assessment, and the Joint Performance Measures, when applicable. 6 / Ensure KS-HAN continues to be up to date. KS-HAN announcements are sent out twice per year as a reminder to update user profiles. Hospital Regional Coordinators should:

  3. Provide assistance to community hospitals in identifying critical staff to be included in KS-HAN. (Capability 6: Information Sharing)
