Annexure-II

TENDER PROCEDURE

The Bidders should submit the proposal in two parts:-

A.  Technical Bid

B.  Price Bid

A.  Technical Bid should contain:-

I.  Detailed composition of Quoted items in prescribed Format as mentioned Annexure – V

II.  Up-to-date sale tax/JVAT Clearance certificate.

III.  Copy of registration with sale tax (Jharkhand) & Income Tax.

IV.  JVAT/Jharkhand sale tax will be deducted at sources.

V.  The Bidder is required to submit audited balance sheet of the firm for last three years (2011-12, 2012-13 & 2013-14).

VI.  Copy of valid Manufacturing license under Drug and Cosmetics Act. 1940 and with the products quoted by the Bidder should be underlined in the VALID Manufacturing license.

VII. 5 (five) years Market standing certificate of quoted products.

VIII.  Bidders are required to submit the paper of in house R&D facilities & copy of paper published in last three years along with technical bid.

IX.  Copy of valid GPM issued by the central Drug laboratory/Drug controller of India / WHO GMP certificate and non conviction certificate from the drug controller of the state concerned.

X.  Sample must be submitted along with tender. Before opening of the price bid of technically qualified tender the packing quality and pack size of the product will be physically verified by the technical committee.

XI.  Tender without Earnest money shall be rejected.

XII.  Bidders campaigning or canvassing or disputing tender condition will render themselves black listed for five years.

XIII. Tender is required to enclose with technical bid and earnest money for an amount equivalent to 2% of the quoted amount in the shape of bank Draft/Bank guarantee of any nationalized bank in favor of Regional Director Animal Husbandry Santal Pargana Range Dumka. Jharkhand.

XIV. Other required documents and notarized affidavits as per prescribed terms & conditions and criteria.

B.  Price Bid

The tenders must indicate the offered price clearly for each packing unit in words and figure only one rate (offer price) should be quoted against one item strictly as per the specifications mentioned.

Price one rate should be quoted without tax and with tax separately for various districts and institutions VIZ Dumka, Jamtara, Sahabganj, Pakur, Deoghar & Godda and Animal Referral hospital Dumka, pet clinic Deoghar, laboratory Dumka. Under Santal Pargana Range Dumka. Rates of different pack size should be quoted separately.

Annexure-III

GENERAL TERMS AND CONDITIONS

1.  Only the bidders in whose name the bid document has been purchased shall submit the bid document. If bid document is down loaded form departmental website the bidder at the time of submission of bid document shall deposit the cost of bid document in the form of demand draft amounting to Rs. 5000/- (Five thousand only) in favor of Regional Director Animal Husbandry Santal Pargana Range Dumka. Jharkhand. Along with the bid envelop.

2.  The bid document purchased from department is not transferable.

3.  The rate quoted will be valid for one year or from the date of rate approved till the next tender.

4.  Place the delivery for various districts VIZ. Dumka, Jamtara, Sahabganj, Pakur, Deoghar & Godda and Animal Referral hospital Dumka, and pet clinic Deoghar, laboratory Dumka. Under Santal Pargana Range Dumka.

5.  Conditional tenders will not be accepted.

6.  Cost of tender document is non refundable.

7.  In case of feed Supplement and digestive tonic cost of recommended per dose should also be mentioned in price bid lowest cost recommended dose will be the basis for comparison of rate quoted.

8.  Overwriting, Cutting should be avoided and be attested. If any.

9.  Every material supplied should have potency/expiry beyond a minimum period of one year from the date of supply otherwise the supply will not be accepted.

10.  The Bidder should assure the supply of the material within 30 days from the date of issue of purchase order.

11.  Cost of materials should not be printed on outer and inner label of the material.

12.  Every drug supplied should be labeled with NOT FOR SALE and Government of Jharkhand supply only.

13.  The drug supplied should bear the label containing the composition in terms of chemical name of the ingredients used for preparation in each and every item and also in accordance with the drug and cosmetic Act. Otherwise the supply will not be accepted.

14.  Should have 5 (five) years market standing certificate of quoted product.

15.  All Bidders must submit their sample with the tender.

16.  Before opening the price bid of technically qualified tender the packing quality and pack size of the product will physically verified by the purchased committee.

17.  If the earnest money is not enclosed along with technical bid the tender will be disqualified.

18.  Payment will be made by the authority issuing the order trough Bank Draft and commission will be borne by the firm.

19.  After verification of EMD amount tenders will be eligible for consideration of their quoted price.

20.  The competent authority reserves the right to place an order for the full or part quantities under any item.

21.  The samples of feed Supplement and digestive tonic will be sent for quality test IVRI Barrelly or BIT Messra Ranchi by the purchaser and the cost incurred on the test will be bear by the supplying firm.

22.  The authority issuing the order reserves right to cancel whole of part of the supply order of those suppliers, who are found to be defaulter for delayed supply as well as sub-standard quality of the materials.

23.  The authority reserves the rights terminate the contract of any agency/agencies in case changes in the government procedure or unsatisfactory services.

24.  Authorization in case person other than authorized bidder has signed Document.

25.  In case of any dispute the legal jurisdiction shall be within the jurisdiction of Dumka only.

ELIGIBILITY CRITERIA

i.  Firm/Manufacturer should be in the market for more than 10 (ten) years.

ii.  Should be holding ISO/GMP Certification for the last 5 (five) years.

iii.  Annual turnover of the firm should not be less than 10 cores in the last 3 (three) years. (2011-12, 2012-13 & 2013-14)

iv.  Should have in house R&D facilities evident from paper published in last 3 (three) years and list of major scientific equipment.

v.  Should have valid CST/VAT Registration No. in Jharkhand.

vi.  JVAT/Jharkhand sale tax will be deducted at sources.

vii.  Firm should have never been BLACKLISTED by any Government of Government bodies & not involved in scam. (Notarized affidavit in this regard)

viii.  Firm should have valid manufacturing license under Drug & Cosmetic Act. 1940 for the products quoted. In case the sole marketing company valid manufacturing license of the third party for the quoted item shall be enclosed along with the tender.

ix.  Should have 5 (five) years market standing certificate of quoted products.

x.  Should have non conviction certificate issued the competent authority.
