PRINCIPLE PURPOSE: To maintain a listing of essential personnel that can be contacted in the event of an emergency arising during off-duty hours.
ROUTINE USES: To contact individual by phone in the event of an off-duty emergency requiring the presence of an on call duty officer to take appropriate action.
DISCLOSURE AND EFFECT ON INDIVIDUAL: Disclosure is voluntary; however, failure to provide the requested data could result in termination of assignment to a key position.
