6) BRIEF RESUME OF THE INTENDED WORK

6.1) NEED FOR THE STUDY:

A normal daily life without moving spine is impossible for any human being.In

the present world, the advancement of busy,professional,social life,improper sitting

postures,continuous and over exertion,jerking movements during travelling and

sports- all these have undue pressure effects on the spine,which results in the most

common disorder called the Low back pain.50 percent of the working population

have back pain every year1.Most of the persons will have radicular pain and this

comes under the Sciatica Syndrome.

Gridhrasi is a shoola pradhanaja vyadhi characterized by ruk in sphik,

kati, uru, janu, jangha, pada and also associated with toda, stamba,

muhurspandana2.The lakshanas of Gridhrasi can be well correlated with the

“Sciatica” in modern.

Ayurveda has explained Shamana line of treatement that includes oral

administration of medicines,Snehana,Svedana,Basti etc.Katibasti is one such

procedure which is not mentioned in our classics,but pacticed by many scholars.

Among the snehanas Prasarini taila3 is indicated in Gridhrasi.

The oral administration of medicine is atmost importance as it is very easy

and also effective.Many herbal combinations described in Ayurveda yet to be

explored.Dvatrimshaka guggulu4 is one such yoga prescribed for vatavyadhis

including Gridhrasi.

In this study an attempt is made to evaluate the comparative effect of

Dvatrimshaka guggulu and Kati basti with Prasarini taila in Gridhrasi.

6.2) REVIEW OF LITERATURE

Gridhrasi is a vata vyadhi characterized by shoola,sthamba,toda,spandana.These

symptoms initially affect sphik,as well as posterior aspect of kati & then gradually

radiates to posterior aspects of ooru,janu,jangha,paada2.This is manifested due to

vatakara ahara vihara.Kati basti with Prasarini taila3and Dvatrimshaka guggulu4

are indicated in the management of Gridhrasi & are said to produce relief of the

condition.

PREVIOUS WORKS DONE:

1)  A comparative clinical study was conducted to evaluate the efficacy of Navjeevanarasa and Rasna guggulu in patients of Gridhrasi.Study concluded with better remission of symptoms in patients treated with Navajeevanarasa5.

2)  A study was conducted to find the efficacy of Rasna guggulu along with Shodana therapy by Virechana followed by Basti karma.The study proved Shodana along with Shamana is efficacious in Gridhrasi6.

3)  A comparative clinical study was designed to conducted to find out the efficacy of Agnikarma and Matrabasti in Gridhrasi.The study revealed that Agnikarma and Matrabasti have got equal effect in Gridhrasi with 44% improvement7.

4)  A single blind clinical study was carried out with the pretest and post test design;where in 20 patients of Gridhrasi were subjected to Kati basti with Vajigandhadi taila for a period of 6days.The result showed best remission of the symptoms in majority of patients, though complete cure was recorded in none8.

6.3) OBJECTIVES OF THE STUDY:

1) To evaluate the therapeutic effect of Dvatrimshaka guggulu in patients suffering

from Gridhrasi.

2) To evaluate the therapeutic effect of Kati basti with Prasarini taila in patients

suffering from Gridhrasi.

3)To evaluate the combined effect of Dvatrimshaka guggulu & Kati basti with

Prasarini taila in patients suffering from Gridhrasi.

7) MATERIALS AND METHODS :

7.1) Source of data:

Minimum 30 patients diagnosed as Gridhrasi will be taken for

study from OPD and IPD of SDM Ayurveda Hospital, Udupi, Karnataka.

7.2) Method of collection of data:

A special proforma will be prepared with all points of history taking, physical

signs, symptoms as mentioned in Ayurveda and lab investigations will be carried

out as mentioned in Allied sciences. Accordingly, patients are selected and will be

subjected to detailed clinical history and complete physical examinations.

Selected patients are randomly placed under three groups so in each group

minimum 10 patients will be included.

Design of the study:

It will be a single blind comparative clinical study with pre-test and post-test design wherein a minimum 30 patients suffering from Gridhrasi will be selected irrespective of their gender, cast or creed. The parameters of signs, symptoms will be scored on the basis of standard method and will be analyzed statistically.

Group A: Administration of Dvatrimshaka guggulu 500mg–2tab thrice a day for 7 days.

Group B: Administration of Kati basti with Prasarini taila once a day for 7 days.

Group C : Administration Dvatrimshaka guggulu 500mg–2tab thrice a day

Administration of Kati basti with Prasarini taila, simultaneously for 7days.

Follow up : 28 days with weekly follow up

Inclusion Criteria:

1)  Patients presenting with Pratyatma Lakshanas of Gridhrasi.

2)  Patients presenting with the signs and symptoms of Sciatica.

3)  Patients between the age of 16 to 60 years.

Exclusion Criteria:

1)  Congenital deformities of spine

2)  Trauma or injury to the spine.

3)  Neoplastic conditions of the spine.

4)  Infections of the spine.

5)  Patients with any other systemic illness.

Assessment Criteria:

Subjective Parameters:

The improvement in the patients will be assessed on the basis of relief in the Signs Symptoms of the disease.

Objective Parameters:

1.Greenough & Fraser Scoring method

2.Herron & Turners Rating

3.Sugar baker & Barofsky Clinical Mobility Scale

4.Oswestry disability Assessment Questionaire

7.3) Does the study require any investigations or interventions to be conducted on patients or other humans or animals? If so please describe briefly:

Study will be conducted on Gridhrasi patients only.

No animal experiments will be done

.

Investigations:

1)  Radiological: X-ray of Lumbo-sacral spine AP & lateral views if

necessary.

2)  Routine : Haematological investigations-Hb%, TC, DC, ESR.

Urine routine and microscopic to rule out other conditions
